SOPs for Safe Injection Practice

1. Purpose
To outline the steps and requirements for performing safe injection practices to prevent
the transmission of infections during medication administration

2. Scope
This procedure applies to all injections, including intravenous, intramuscular, and
subcutaneous at health care facilities

3. Responsibilities
All healthcare workers (HCWs) that provide injectable medicines

4. Required Standards
 Injections should only be performed by licensed HCWs
 Always use aseptic technique
 Keep the injection preparation area free of clutter
 Ensure that the patient is adequately prepared for and informed about the
procedure
 Check the drug chart or prescription for the medication (all rights), correct
medication at correct dosage, correct patient, correct request, etc.

5. Equipment required
 Injectable solution (e.g., medication) with diluents as appropriate
 Sterile syringe with needle
 Alcohol swab or cotton wool ball and 60%-70% non-expired alcohol solution
 Sterile blind hubs
 Disposable cardboard / cleanable tray
 Sharps container for sharps disposal
 Emergency drugs as needed

7. Key safety rules


 ALWAYS follow the one needle, one syringe, and one injection rule
 DO NOT use a single-loaded syringe to administer a medication to several
patients, even if you change the needle every time between patients
 DO NOT use the same mixing syringe and needle to reconstitute several vials
 DO NOT combine leftover medications for later use
 DO NOT use single-use vials for multiple patients

8. Procedure for Administering Injections

1. Clean the working area. Select and check equipment. Gather equipment. ...
2. Perform hand hygiene with soap and water.
3. Prepare a well-laid up tray including emergency drugs for management of
possible drug reaction.
4. Unfold the sterile pack. Open any other equipment. Arrange the pack contents on
the sterile field. ...

6. Wash hands with soap and water and dry up hands with air or towel.
7. Alcohol could be used as a secondary step after soap and water except for EPI
injection.
8. Check for the integrity of the vial/ampoule for the following expiry date, breach,
leaks, particles, or any contamination.
9. Make sure that the right dose, formulation, and route are used for the right
patient or client.
10. For medications that need to be reconstituted (powder forms), it should be done
according to the manufacturer’s instruction and use the correct diluents.
11. Draw the right dose as prescribed, including expelling the air and using right
injection equipment.
12. Ensure aseptic technique while giving the injection.
13. Administer the drug at the correct site
14. Don’t recap needles if applicable
15. Dispose the used syringes and needles immediately into the sharp’s container.

17. A patient should be kept in the room for at least 5 minutes after the injection has
been given and be observed for any possible adverse effect or events.
18. Record the date and time of injection administered.
