1. Subject
During yard monitoring, found the visual welding problem, Slag inclusions and
irregular weld beads as the below photo.
2. Investigation

3. Conclusion of Investigation
1) All welding conditions to be investigated are met the general welding
requirements except the inter pass cleaning and wind protection.
2) Bad workmanship and skill
Welder (ID No.: LJ 201) has a bad workmanship, he performed welding without inter
pass cleaning/grinding to remove slag and to rectify a bad groove profile and wind
protection that can cause slag inclusion and/or porosity. And he performed the bad
welding continuously without any corrective action even though he recognized that perform
the poor quality welding.
LJ 201, welder is a qualified welder with SMAW process at 6G position for SUS pipe
on Jan. 30, 2015 and an experienced welder, and his skill is proved with the result of the
opposite side welding.
4. Corrective Action
1) CPH, Welding engineering team instruct all welders regarding the below matters,
- Inter pass cleaning and grinding to remove the slag and to rectify the bad weld
groove profile.
- Install the wind protection kids when perform welding at out door.
- Keep the dry condition of welding electrodes, store welding electrodes in the hot
quiver.
- Remind that welder’s responsibility, welders must guarantee not only the quality
of the surface of weld but also the soundness of fill pass(s) of the weld for the
partial joint penetration and complete joint penetration even though not apply
UT or RT.
2) The related weld joint will be repaired; all existing welding discontinuities will be
removed by grinding and inspected by CPH QC inspector before repair welding.
