Dispensing of prescriptions

1/ check the legality, validity & completeness of the prescription

2/ Interpret Medication abbreviation

Example - gtt – drop

Pc – after meal

Qd – every day

Hs – at noct time or at bed time

Bid – two time day

3/ Depict the Indication and MOA of the drugs

Example – Rx- Cotrimoxazole 480mg tablet po Bid for 7 days.

- Indication – Cotrimoxazole antibiotics mainly used for pneumonia corni


pneumonia (PCP).
- MOA – Inhibit the folic acid synthesis of bacteria by the inhibiting the PABA.

4/ calculate the quantity of each drug.

Example :- Rx - Insulin 20/30 I.U Bid for 2 month

- Ketoconazole 200mg tablet po daily for days

Insulin – 20x30 =-50 I.u per days

50x 60 = 3000 I.u

1 vial = 100 I.u/1ml or 1000 I.u/10ml

1vial = 10ml

1vial = 1000 I.u

? x = 3000 I.u = 3 vials

Ketoconozole - 1x20 = 20 tablets

5/ determine price of each drugs

Example – Rx- Clarithromycin 500mg tablet po Bid for 7 days


price 1 tablet clarithromycin = 3 birr

2x7 = 14 tablets

1 tablet = 3 birr 14 x 3 = 42 birr

6/ Detect and respond to drug Interaction & contraindications

Example – Rx- Frusemide 40mg tablet po daily for 10 days

- Gentamycin injection Bid for 14 days


Gentamycin and Frusemide Interact each other

Drugs- drug Interaction- Gentamycin and frusemide interact each other both of them
have a side effects of ototoxicity. Potentiate ototoxicity side effects.

Ototoxicity.

Contraindication – Gentamycin – for Renal failure patients.

Rx – Tetracycillin 500mg capsule po daily for 7 days

- Antacids suspension 10ml Tid for 10 days

Drugs- drugs Interactions- TTC Indirect with antacid in the absorption

- Antacid decreased the absorption of TTC.


- Our advice should be to use time gap.

Drug – food Interaction

- TTC & Antacid Interact with foods b/c of this must be used before 1 or 2 hours
or after 1 or 2 hours meals (foods.

Contraindication – TTC – contraincated for pregnant women

Lactating mother

Children < 8 year

7/ provide product specific counseling

Example: - Rx citrizine 10 mg tablet po daily at noct time for 10 das.

- Doxycillin 100mg capsule po Bid for 10 days.

Counseling – for citrizine to take at night or Bed time because of their sedation side
effects.

- Doxycillin – to used 12 hours gap. ( two times a days

50 x 60= 3000 I.u

1 vial + 1000 I.u

Omeprazole – 2 x7 = 14 capsule

Clarithramycin – 2 x 7 = 14 capsules

Amoxacillin = available a 500mg so need two capsule for one administration.

2x2x7 = 28 capsule

 The use of insulin is for Diabetic patients especially for types I DM. b/c type
one is Insulin dependent diabetes millitus.
 Insulin is secreted by pancreatic Beta cells.
 Stored in refrigerator. / cold storage /
 Have hypoglycemia side effects due to -over dose
-Exercise
-Using other oral hypoglycemic drugs.
- Alcohol.

- Also have liptrophy side effects – damaging lipid cell in the injection site. We
can avoid these side effects by administering different sites.
- In morning the dose is not high b/c there is no glucose more in our body.
- In evening the dose is higher (greater than the morning) b/c there is more
glucose in our body.
- Omeprazole, Amoxicillin, clarithromycin this all drugs are used for ttt of PUD
associated by Helico bacter pyroli.
- Also known as triple therapy.

2/ Rx – PPF 600,000 I.u daily for 7 days

- Ferrous sulfate tablet take as before


- Neomycin 500mg tablet Bid for 7 days
- Paracetamol 500mg prn = 10 tablet

1/ PPF = 600,000 I.u x7 = 4, 200,000 I.u water for

1 PPF vial = 4,000,000 I. u injection

? = x = 4,200,000 I.u 10m = 1 vial

1 x4 , 200,000 = 1.05  1vials 5ml= 2 vial

Ferrous sulfate – Is not dispensed b/c it says take as before.

Neomycin – 2 x7 = 14 days paracetamol = 10 tablet or 1 strip.

Comment

-PPF after reconstituted stored at refrigerator.

- PPF is penicillin group is antibiotics.

- PPF given by IM injection.

- Ferrous sulfate and Neomycin are Interact each other ferrous sulfate decrease
absorption of Neomycin.

- Ferrous sulfate – used for Iron deficiency.

- Neomycin – Antibiotics Is Amino glycosides group

- Inhibit the protein synthesis of the bacteria

- Amino glycosides are highly polar b/c of this the oral absorption is less.

- But Neomycin is available in tablet forms for local effects. Have bactericidal effects.

- group of Neomycin are like Gentamycin, streptomycin

- paracetamol – Is non-opoid analgesics .

- have antipyretics & analgesics effects

Compounding: - mix magnesium sulfate and light magnesium carbonate in mortar


and pestle and triturate the mixture with some water. Transfer the mixed mixture to a
measure and double strength chloroform water and add concentrated peppermint
emulsion. Finally adjust to the final volume with the water.

Action and use – laxative

Reducing the formula

- Magnesium sulfate = 400 g __ 1000ml 400 x 25

- Light magnesium carbonate = 50 – 1000ml = 50 x 25= 1.25g

- Concentrated pipperment Emulsion = 25 – 100ml = 25 x 25 = 0.6ml

Compounding – weight the amount of Benzoic Acid, salicylic acid and white
petroleum and put on the ointment tile then triturate each other by taking small
amount & levigate two or three times until grity particle are not shown then transfer to
the ointment jars.

Action and uses – Anti fungal.
