PHAR1006 Written Calculation OSCE 2024 - Practice Tests

Practice Test 1

1. A pharmacist is asked to assist in determining the correct dose of epoetin alfa injection for a
76-year old male patient who weighs 75kg. The patient’s initial haemoglobin is 9.2 g/dl. The
reference (Australian Medicines Handbook) states the starting adult dose of epoetin alfa for
anaemia of chronic renal failure is “50 to 200 units/kg administered IV/SC three times each
week”.

Calculate the total volume (in mL) to be administered during the first week of treatment if
the patient is to start at minimum starting dose and epoetin alfa is available as a 10,000
units/mL injection. Round your final answer to three decimal places.

Minimum starting dose per week for epoetin alfa

50 units X 75 X 3 = 11250 units

Epoetin alfa available as 10,000 units/mL

Cross multiplication to get the total volume to be administered per week:

11250 𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢
X 1mL = 1.125mL (Round answer to 3 decimal places)
10000 𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢

2. Calculate the oral dose of methotrexate (in mg) suitable for a 5 year old child (weight 21kg
and height 1.3m) if the oral dose of methotrexate is 15mg/m2. Provide your final answer as a
whole number.

BSA = Sqrt of [(130 x 21)/3600] = 0.8708 m2 (Keep as many decimal places as possible in your
calculation steps before you reach the final answer)

Daily Dose = 15mg/m2 x 0.8708 m2 = 13.06mg = 13mg (Answer as a whole number)

3. How many drops would be prescribed in each dose of a liquid medicine if 15mL contained 60
doses and the dispensing dropper calibrates 28 drops/mL? Provide your final answer as a
whole number.

Volume of 1 dose = 15mL / 60 doses = 0.25mL

28 𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑 𝑋𝑋 0.25𝑚𝑚𝑚𝑚
Therefore, number of drops per dose = = 7 drops
1𝑚𝑚𝑚𝑚

Practice Test 2

1. If a 5mL vial contains 300 units of abobotulinumtoxinA (Dysport) and 0.1 mL is injected into
each of six sites during a procedure, how many units of drug would remain in the vial?
Provide your answer as a whole number.

Volume of Dysport left in the vial after the procedure = 5mL – (6 x 0.1mL) = 4.4mL
4.4𝑚𝑚𝑚𝑚 𝑋𝑋 300 𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢
Number of units left in the vial = = 264 units
5𝑚𝑚𝑚𝑚

2. An intravenous infusion contains 10mL of a 1:5000 solution of isoproterenol hydrochloride

and 400mL of 5% dextrose injection. If the medication needs to be administered at 5 µg per
minute, how long will it take to complete the infusion. Provide your answer as a whole
number in minutes.

10mL of 1:5000 solution contains 2mg of the drug.

(10mL/5000 = 0.002g = 2mg = 2000 µg)

Therefore, 2000µg of drug in 410mL of infusion

Volume of infusion that deliver 5µg:

2000µg/5µg = 410mL / x mL

X = 1.025mL

3. Medication order calls for 400mg of clindamycin to be added to 600mL of Dextrose 5%

water for IV infusion over 90 minutes. Clindamycin is available as an injection containing
600mg/4mL. Calculate the required rate of flow in millilitres per minute. Round your final
answer to one decimal place.
400𝑚𝑚𝑚𝑚 𝑋𝑋 4𝑚𝑚𝑚𝑚
Volume of clindamycin to be added = = 2.667mL
600𝑚𝑚𝑚𝑚

Total volume of infusion = 600mL + 2.667mL = 602.667mL

Rate of flow in mL per minute = 602.667mL/90 minutes = 6.7mL/minute

Therefore, rate of administration = 1.025mL / minute

Duration of infusion = 410mL/1.025mL/min = 400 minutes

Practice Test 3

1. You receive a prescription for benzylpenicillin suspension as below

Rx:
Supply 40mL of Benzylpenicillin 15,000 units/mL

You have benzylpenicillin available in a vial containing 20,00,000 units of the dry powder in
the pharmacy. The directions on the vial are to add 42 mL of sterile water for injection to
obtain a concentration of 45,000 units per millilitre. How many millilitres of the
reconstituted solution should be withdrawn to prepare the prescription? Round your final
answer to one decimal place.

Amount of Benzylpenicillin required as per Rx = 40mL X 15000units/mL = 600000 units

Concentration of reconstituted solutions available in pharmacy = 45000 units/mL

600000 𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢 𝑋𝑋 1𝑚𝑚𝑚𝑚
Volume of reconstituted solutions to be withdrawn = = 13.3mL
45000 𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢𝑢

2. A patient (height 1.75 m, weight 75 kg) is being treated for breast cancer with doxorubicin
60 mg/m2 on day 1 of each 21-day cycle. What dose of doxorubicin (in mg) is administered
on day 1 of the cycle? Round your final answer to one decimal place.

BSA = Sqrt of [(175 x 75)/3600] = 1.9094 m2 (Keep as many decimal places as possible in your
calculation steps before you reach the final answer)

Daily Dose = 60mg/m2 x 1.9094 m2 = 114.6 (Round answer to one decimal place)

3. 20mL of 10% calcium gluconate injection and 45mL of multivitamin infusion are added to
500mL of a 5% dextrose injection. The infusion is to be administered over 4 hours. If the
delivery set calibrates 96 drops/4mL, at what rate (drops/minute) should the flow be
adjusted. Provide your answer as a whole number.

Total volume of infusion = 20mL + 45mL + 500mL = 565mL

Rate of infusion in mL/minute = 565mL / (4X60) minutes = 2.3542 mL/minute

2.3542𝑚𝑚𝑚𝑚
𝑋𝑋96 𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑𝑑
Rate of infusion in drops/minute = 𝑚𝑚𝑚𝑚𝑚𝑚𝑚𝑚𝑚𝑚𝑚𝑚
4𝑚𝑚𝑚𝑚
= 57 drops
