Introduction

Effective communication is critical for delivering quality and safe patient care, especially in

health care, with its many professional interfaces. The scenario above identified potential deficits

in using the ISBAR routinely for handoffs at a cardiac rehab facility. As a current nurse leader in

the same facility, I know this situation requires immediate redress and remedial measures toward

better interprofessional communication and collaboration.

Investigating the Issue

As a nursing leader, these are the steps I would take to investigate this issue further further. I

would first observe how handoffs are done from one nurse to another and between nurses and

other medical health professionals to assess the uniformity level in implementing the ISBAR

tool. This will help me pinpoint some problem areas and possible communication barriers. It

would also include interviews with the nursing staff and students to get their views on the

ISBAR tool. Their responses would provide insight into their knowledge level, confidence levels,

and challenges in applying the ISBAR tool. I will also review the policies and procedures of the

facility for handoff communication, including the guideline procedures and protocols in the

application of the ISBAR tool. This review would help me to find gaps or inconsistencies in the

established protocols. Last, I would review any incident reports or quality improvement data

about the communication breakdown or errors in the handover process. This information might

shed light on the impacts of poor communication and the need for change.

Addressing Inconsistent Use of ISBAR

If my audit showed that the team members were not using the ISBAR tool appropriately, the

following would be my actions as a leader nurse: I would come up with and deliver an

interactive, case-study-enhanced, practical-with-feedback, comprehensive educational and


training program on how to use the ISBAR tool appropriately for all staff and students. Besides

that, I also urge the importance of effective communication in patient safety and good quality of

care. That could be executed by holding staff meetings and posting visual cues around the

facility to remind employees of key policy or process changes.

Providing Education on ISBAR

It is prudent for a nurse leader like myself to educate the nursing students and staff on using the

ISBAR tool. The following are some reasons why education on using the ISBAR tool is

essential. The education on the use of the ISBAR tool will enable me to bestow an understanding

on all team members, including students and staff, on the application of the tool. This is crucial

to ensure that communication among the interprofessional members is effective and

collaborative. Second is the fact that handoff communication is a high risk to patient safety.

Proper education to all the team members on how to apply the rules of ISBAR will reduce such

risks of communication errors, which may cause adverse events and probably harm to the

patients. Third, the education provided on communication tools, specifically ISBAR, is

considered professional development for nursing students and staff. It will enhance the

knowledge and skills that are required of them in providing quality care to patients. The last but

most important is that as a nurse leader, I am supposed to see that the nursing students are

correctly oriented when they undertake clinical placements. Easy linkage can be achieved with

this responsibility and the nurse preceptor's role in education on ISBAR.

Conclusion

This will be an effective practice to enhance efficient interprofessional communication and

collaboration in a cardiac rehabilitation facility, where many health professionals are used. As a

nursing leader, I will take a stand against the inconsistency of the ISBAR tool in communication.
We should uplift such a practice by raising the issue, standardizing training, and re-educating on

the importance of practice. This, in turn, will create an open communication culture, enhancing

patient safety, teamwork, and professional development. Other future steps are monitoring,

refresher training, a technology-based system, and standardized handoff protocols to improve to

improve communication and collaboration.
