Introduction

This scale is designed to rate the severity of obsessive and compulsive symptoms in children

and adolescents, ages 6 to 17 years.

Reason of referral

Subject was selected for the purpose of psychological assessment.

Background information

Family history

Subject's mother was a housewife. The subject had a healthy relationship with mother. The

subject's father was a business man and she had a good relationship with father. Subject has 4

siblings. A younger sister is a doctor and works in a clinic but her elder sister studied in school.

She is very close to her elder sister and can share everything with her. She can share her personal

problem with her elder sister. Subjects also have healthy relationships with other siblings.

Overall,3the atmosphere in the house was healthy.

Personal History

The subject's birth was normal. All milestones were achieved in the normal age range. The

subject started school at the age of 5. Her overall class performance was good. She was an

average student. She participated in co-curricular activities in school. Her hobbies were watching

TV, cooking, reading and writing a poetry. Client was delivered by normal delivery in a public

hospital and had immediate crying prior to delivery. Client’s mother reported that she did not

have any complications during or after pregnancy. She was in good health and the client was also

a healthy child at birth. Her birth weight was normal at the time of birth. The client’s

developmental milestones were reported to be age appropriate by mother. There were no delays

in neck holding, babbling, crawling, walking, speech, etc. No neurotic traits were observed and

reported in the client.

Educational History

The client was admitted to school when she was 5 years old. According to the client's mother, the

client was academically average and did not adjust well with her peers and teachers. She was

more interested in extracurricular activities in her school and participated in sports. She

socialized well with peers at school and elsewhere, but often felt that no one liked her, so she

could sit alone in class all the time because people looked down on her. She assumes that

everyone ignores her and doesn't understand her. she felt that everyone was teasing her.

Then her college life went well she got average marks in intermediate and got admission in

university she thoughts regarding people are still like her school. She thinks everyone ignores

her. So she were spend a lot of time around people because of her fear of being alone, she felt

that she will be alone if she doesn't talk to them. She believed that people would judge her

negatively and this affected her academic life as she became confused and nervous in front of

everyone. Then she gets admission in university and she tries to strengthen her friendship with

new people, she stays with them all the time and shares her personal problems with them and

feels good.

Assessment procedure

This test can be given to people above 16 years of age. As much time is required as the subject

wants to cover, don't feel pressured to finish the topic in the specified time. It depends on the

person's choice how much time they complete the test. A person sitting comfortably should

ensure that there are no distractions behind them so that they are actively paying attention. The

subject is not distracted by any unusual objects and sounds of people.

Test administration

The time taken by subject for the test is one hour. The test was conducted in the room. All

material provided by the examiner. The examiner instructs him as per the manual. Subject

follows instructions. Subject asked several questions before the test. The examiner answers each

question until his confusion is cleared.

Behavioral Observation

The subject was neatly dressed up. The subject was sitting comfortably in a chair. her expression

seems curious before the test. her expressions during the test were vague. she showed

cooperative behavior throughout the session. The subject was 24 years old and had an average

height and body weight. She was maintaining eye contact during the session. Her personal

hygiene was satisfactory as she wore clean clothes.
