GENERAL
If there is area rented to other companies within the same facility, please provide the relevant Rent or

Personnel files (including employment registration record, photocopy of ID card, passport or legal
5 identification, Employment letters / Labour contracts, social security coverage documents etc.) and/or
Collective Labor Agreements (if applicable)
Permit for engaging labor agents, business license of labor agent (if applicable), contract signed with
6 labor agent and if foreign workers employed, also provide official employment permit granted by local
authority.
Occupational health examination records (wherever applicable like workers engaged in hazardous

operations, canteen, young workers etc)
Factory regulations, employee manual and labor policies (including the policies for working-hour
system, overtime compensation and other legal benefits, hiring policies/procedure, anti-corruption,
8
child labour, young workers, forced labour, non- discrimination, harassment & abuse, freedom of
association, security guards responsibilities, dormitory rules etc.)
10 Payment receipts and all related documentation on coverage of social insurance schemes
Wages and benefits records of the most recent consecutive 12 months and the corresponding time
11 attendance records (including all active employees and resigned employees). If wage paid by bank
transfer, bank transfer record is required.
Dispatch agreement, collective bargaining agreement (if any), internship agreement (If dispatched

workers and interns are used)
Grievance procedure and workers complaint records, minutes of all worker committee meetings, all
14
worker & staff trainings on company policies mentioned in point # 8.
Most recent social compliance audit report and/or governmental labor/fire inspection report and
15
corrective action plan, with the current status of each finding
Production records (e.g. daily production records, piece-rate records, material in/out records, QC
16
inspection records, broken needle records, etc.)
Chemical inventory, material safety data sheet (MSDS) for hazardous substances/chemicals (if
17 applicable)
21 Firefighting equipment list and inspection records, machine & electrical maintenance records
23 Meeting records and minutes of environmental, health & safety committee (if any)
24 First aid box contents list, first aid training & first aider certificate
All training records for workers as well as implementing staff members (Environmental, Health &

Safety (EHS), chemical handling, fire safety, PPEs etc)
Work permits for special and dangerous operations (e.g. electrician certificate, forklift operator
26 certificate, welder certificate, boiler operator, cargo lift operator, air compressor operator,
crane operator, generator installation permission etc., if applicable)
Safety certificates &/or inspection reports for special appliances (e.g. for elevator, boiler, air

compressor, crane, etc., if applicable)
28 Canteen hygiene certificate, drinking water test reports, pest control records (if needed)
30 Hazardous waste disposal unit license & manifest for hazardous waste disposal
Environmental protection documents (if applicable) like environmental policy, all environmental

permits, test reports, training records etc.
OTHER DOCUMENTS
List of subcontractors (As defined in WSI standards) & signed acknowledgement of WSI Vendor
32 Code Of Conduct (COC) from each sub contractor that is used for WSI production, and also any
records of applicable COC training /audit on subcontractors
Record of local community services and/or collaboration project with Non-Profit Organization
