1. Know the reason why the client is receiving the medication, the drug
classification, contraindications, usual dosage range, side effects, and
nursing considerations for administering and evaluating the intended
outcomes for the medication.
2. Check the MAR ( Medication Administration Record)for drug name,
dosage, frequency, route of administration, and expiration date for
administering the medication, if appropriate.
3. Review medication reference information for medication action,
purpose, normal dose, side effects, time and peak of onset, and nursing
implications.
4. Organize the supplies for subcutaneous injection.
5. Perform hand hygiene.
6. Obtain the appropriate medication for the patient.
7. Compare the label of the medication container against the order on the
MAR.
8. Check the expiration date of the medication.
9. Prepare the medication: calculate the medication dosage accurately and
prepare the correct amount of medication for the required dose.
10. While preparing the medication, recheck each prepared medication and
container with the MAR again.
11. Identify the patient using at least two identifiers, check it from the
patient’s MAR.
IMPLEMENTATION
12. Perform hand hygiene and wear clean gloves.
13. Positioned patient comfortably for site assessment. Select appropriate
site. Inspect skin surface over bruises, inflammation, or edema. Do not
use an area that had signs associated with infection.
14. Palpate sites and avoid those with masses or tenderness

15. Ensure that needle is in correct size by grasping skinfold at site with
thumb and forefinger. Measure fold from top to bottom. Make sure that
needle is one-half length of fold.
16. Keep patient in a comfortable position. Ask him/her to relax while
locating the appropriate site for injection.
17. Clean site with antiseptic swab. Apply swab at the center of the site and
rotate outward in a circular motion for about 5cm (2 inches)
18. Hold swab or gauze between third and fourth fingers of non- dominant
hand.
19. Removed needle cap or protective sheath by pulling it straight off.
20. Hold syringe between thumb and forefinger of dominant hand. Hold it
as a dart.
21. Administer injection (via syringe):
a. For an average size patient, hold the skin across injection site or
pinch skin with non- dominant hand.
b. Inject needle quickly and firmly at 45 angle. Release skin if
pinched, when using an injection pen or giving heparin continue
to pinch skin while injecting the medication
c. For an obese patient, pinch skin at site and inject needle at a 90
degrees angle.
d. After needle injected, grasp lower end of syringe barrel with
non-dominant hand to stabilize it. Move dominant hand to end of
the plunger and slowly inject medication.
e. When giving heparin, inject over 30 seconds, avoid moving
syringe.
f. Withdraw needle quickly while placing an antiseptic swab or
gauze gently over site.
22. Document each medication given. Record the medication given, dosage,
time, any complaints or assessments of the client, and your signature. If
medication was refused or omitted, record this fact on the appropriate
record; document the reason, when possible, and the nurse’s actions
according to agency policy.
23. Dispose all supplies appropriately. Discard used disposable supplies.
