DECLARATION
1. I PRITI JHA son/daughter of ARJUN JHA request for permission to present myself at the Intermediate Examination to be held in MAY 2020 by the Institute of Chartered Accountants of India and hold valid registration
for appearing in Intermediate Examination to be held in MAY 2020.
2. I declare that I am eligible to appear in the Intermediate Examination as I have passed the PEE-I / Foundation(Prior to May/June 2019)/Entrance Examination and have completed eight months study course in
Intermediate; Or
3. I have passed the Common Proficiency Test (CPT) as well as Senior Secondary (10+2) Examination or an examination recognized as equivalent thereto by the Central Govt. and have registered for Intermediate on or
before 01st September, 2019 ; Or
4. I have passed the Foundation(in May/June 2019) as well as Senior Secondary (10+2) Examination or an examination recognized as equivalent thereto by the Central Govt. and have registered for Intermediate on or
before 01st October, 2019 ; Or
5. I had already registered for the erstwhile Intermediate Examination as per syllabus under Paragraph 2 or 2A of Schedule B of Chartered Accountants Regulations 1988 or Professional Education (Course II) under sub-
regulation (1) of regulation 28A or Professional Competence Course under Regulation 28 C of the Chartered Accountants Regulations, 1988 and have already converted into the Intermediate; Or
6. I registered/applied for Intermediate course/ATC on or before 30th June, 2019 under the Direct Entry Scheme, under the Graduate/Post Graduate category and completed/would be completing 6 months of article ship
as on 01st May, 2020 ; Or
7. I registered for Intermediate course/ATC after 30th June, 2019 under the Direct Entry Scheme, under the Graduate/Post Graduate category and completed/would be completing 9 months of article ship including 8
months of study course as on 01st May, 2020 ; Or
8. I converted to Intermediate under the Direct Entry Scheme, under the Graduate/Post Graduate category from erstwhile Intermediate or PE-II or PCC or IPCE or Intermediate (IPC) and completed/would be completing
9 months of articleship( including 8 months of study course) as on 01st May, 2020; Or
9. I registered for Intermediate under Direct Entry Scheme, having passed the Intermediate level examination of the Institute of Cost Accountants of India/The Institute of Company Secretaries of India, on or before 01st
September, 2019 and completed or would be completing 8 months study course as on 01st May, 2020 ; Or
10. I converted to Intermediate from erstwhile Intermediate/PE-II/PCC /IPCE or Intermediate (IPC), under the Direct Entry Scheme, having passed the Intermediate Level examination of The Institute of Company
Secretaries of India/Institute of Cost Accountants of India, and completed/would be completing 8 months of study course, as on 01st May, 2020 ; Or
11. I registered for erstwhile Intermediate (IPC) course through CPT route and appeared in the erstwhile Intermediate (IPC) Examination in one or both groups but did not qualify both the groups or either of the groups
and converted to erstwhile Intermediate (IPC) under the Direct Entry Scheme and now converted to Intermediate and commenced / would be commencing practical training on or before 30th April, 2020./ I registered for
Intermediate course through CPT/FOUNDATION route and appeared in the Intermediate Examination in one or both groups but did not qualify both the groups or either of the groups and now converted to Direct Entry
Scheme and commenced / would be commencing practical training on or before 30th April, 2020.
