Appointment Letter - Sample - Sakhiya Skin
The Company will appoint you as a "UNIT HEAD" of the Department called “DIPLOMA IN
DERMATOLOGY AND VENEREOLOGY”, which may remuneration:
1. You will be entitled to receive Rs. 25,000/- on monthly basis as Honorarium on and
above his/her routine remuneration.
2. This offer is subject to acceptance from your end, clearances of your Reference
check, Documents verification and Background Check.
3. You are appointed for the Full time UNIT HEAD for LAL DARWAJA branch.
4. Teacher have to take required training and upgrade herself/himself to fulfil his
duties.
Include performing other associated functions that may be assigned to him/ her from time
to time.
2. TERM OF AGREEMENT: Shall be for a period of (2) Two Years from the execution
of this Agreement.
3. TERMINATION:
That, if at any time during your employment, you are found guilty of misconduct or any
willful breach or continuous negligence of the terms of this agreement or dereliction of
the duties and/or instructions given to you from time to time by the Company, the
Company may without prejudice to any other action as may be called for, without two
months notice or payment in lieu of any notice, put an end to and determine the
employment with the company, without prejudice to the above. You shall be deemed to
be breach of the terms of this agreement and further deemed to have brought about
such a situation by your misconduct compelling the Company to put an end to your
services.
4. BREACH OF AGREEMENT:
You shall be deemed to be in breach of the agreement if any one or more event may
occur:
a. Negligence at work.
b. Guilty of misconduct or any willful breach or continuous negligence of the terms of this
agreement or dereliction of the duties and/or instructions given to you from time to time
by the Company.
5. UNDERTAKINGS:
That you shall not take up whole time or part time employment with any other company
engaged in the similar business, for a period of Two (2) years from the date of this
agreement If you break bond without prior 2 months of notice period then you have to pay
quantified specific damages and fulfilling all conditions as stipulated in this agreement.
6. CONFIDENTALITY:
You agree not to divulge directly or indirectly, any information, trade secrets and other
confidential information, received by/ from the company if any, as per the restrictions to
"disclosure of the information". You also agree not divulge to any person, firm or make
public any of the secrets, accounts, transactions, knowledge of Intellectual Property or skill,
acquired from the company.
7. JURISDICTION:
Terms and/or any disputes arising out of this Agreement shall be subject to the Jurisdiction
of Courts in Surat.
DECLARATION:
You hereby declares that this agreement has been entered into and executed by you on
own free will and without any coercion or duress of any nature whatsoever. This
agreement shall continue to remain in force even if you is transferred to any sister concern
of the Company in other location as acceptable to you. It is clarified that the tenure of the
employment service under such sister concern will be taken into account for the purpose of
reckoning the time under this agreement. The agreement is executed in two counterparts.
Each counterpart shall be deemed to be original agreement. Both the parties shall retain
one copy of the Agreement.
IN WITNESS WHEREOF the parties hereto have set their hands to this agreement on the
day, month and year first above written.

General Agreement
This is the general agreement between Sakhiya Skin Clinic Pvt. Ltd and
2) You have to work for more then Nine (9) Hours in a day in Festival season
Whenever requirement.
3) Considering requirement, You can not take leave without intimation &
Approval and same department two staff/Employee can not take leave at
same time.
4) You have to submit mobile phones to the Centre head during your
working hours.
5) You will be eligible for paid leave (CL & SL) after completion of your
probation period (3 Months). Any leave within probation period will be
count as a absent.
8) For the security purpose, Your one month salary will be keep hold
by the company in form of 40% from first month salary, 30% from
second & third month salary and hold salary will be reimbursed to
you after completion of agreement period. (2 Years).
I have understood above terms and condition, i hereby giving you the surety that i
will follow all above terms and conditions in proper manner.
