Reminders: POLICIES and REGULATIONS.

 NO Orientation, NO Normal Laboratory results----NO DUTY!


 Always greet with a smile! To Doctors, Staff Nurses and other health workers, security, utility, and ward maid.
 Orthopedic ward is at 2ndfloor, opposite wing of OB ward.
 Ward 2/ORTHO with bed capacity of 29 beds + 11 beds at veranda = 40 beds.
 Ward 3/ENT has 6 beds located near the elevator.
 Conference area is located in front of the casting and dressing room at 2ndfloor.
 NO Cellular Phones allowed on Duty. If you need to bring it on duty it must be on silent mode and only
emergency and necessary usage is allowed. The Team Leader will be held responsible for keeping your phones.
During snacks and lunch, you are not allowed to play games and scan through social media.
 No standby allowed inside of the Nurses Station, only if you are asked to go in. If you need so see the chart
kindly ask permission from any staff and use the table in front of the nurses station.
 OPD is from Mon-Friday 8am-2pm at veranda. 2 SN will be assigned daily to assist.
 Clubfoot clinic is on Saturday from 8am-11am, 2 SN will be assigned to assist.
 Ortho Ward. Please be on time. 5:30-5:45 is sign in time. 5:45am is endorsement.
 Shifting is 6am-2pm, 2pm-10pm, 10pm-6am.
 Wear your complete uniform. Name Tag and school ID.
 Use only white, black or silver wrist watch with 2nd hand. No digital wristwatch.
 Bring your kits, RLE notebook, small notebook, ballpen black/blue/red, basin for bedbath.
 Use your own BP app, thermometer and pulse oximeter. Disinfect every after use from each patient.
 Always wear face mask. (white color)
 Provide your own clean gloves. Every contact to patient must wear clean gloves. Do not reuse.
 Better to bring box of clean gloves per group for your consumption.
 Bring your own alcohol for disinfection and hand rubbing.
 Use the sink for handwashing and observe minimum usage of water.
 Use your OWN HAND TOWEL or tissue to dry your hands.
 CR is located at the Nurses Station, please have the manner to keep it clean at all times.
 Do not squat or step on the bowl. Keep the floor dry, avoid spilling the water.
 Throw your trash properly and sanitary pads must be wrapped with tissue before putting inside the bin.
 Bring your OWN TISSUE to wipe your hands and do after care.
 If you can’t follow, then you are free to use the CR occupied by the patients and watchers in the ward.
 Bring extra water, snacks, candies. Water dispenser is provided but we need to limit the consumption and at
least pay little amount for the refill.
 Stay within the premises only, no loitering around. Ask permission if you will accompany staff for transport.
 Take note of the name of the Senior Nurses on duty! You will need their names during signing of clearance.
 Please Secure your Clearance every last day of your duty in the area. (per Petro protocol)
 Signing of clearance is done on the last day of rotation between 8am-5pm. In case you failed to do so, you must
come to the area wearing appropriate clothing with lab gown and ID. Mon-Fri 8am-5pm only!
 Please observe and be mindful of the surrounding, not all rules and policy are stated. It is your initiative to act
accordingly and handle the situation and be responsible of your action.

Student Nurses on Duty:

 Please enhance your knowledge, skills and attitude in every procedure.


 A short orientation about physical setup will be done on the 1st day of your duty.
 3-days duty will be all hands on and your critical thinking and decision making is very important.
 You will be taking vital signs, I and O and giving medications to all patients independently.
 You will handle patient with charting on: first day is 1:1 then 2nd and 3rd day is 1:2. (Per approval of NOD)
 Charting (nurse’s notes, VS, medication, I and O)
FDAR color coding per shift…am-black, pm-blue, night-blue.
VS follow legends. Do manual counting for RR and PR.
 You will also be assigned accordingly, each day with different task.
 2 Team leaders will be assigned daily.
 2 vitals sign SN will be assigned daily.
 2 medication SN will be assigned daily.
 2 OPD SN will be assigned daily. Or Clubfoot Clinic SN during Saturday.
 Borrow chart politely from the Staff and Doctors and use the long table outside the station.
 Every procedure must be guided and supervised by the CI or TL, always ask if in doubt.
 IV cannulation is allowed for 2 attempts only with CI supervision.
 IV termination must observe aseptic technique. Press the site for 5min to prevent bleeding.
 Skin test for IV antibiotics and other meds. 1:9 dilution. 30mins for result.
 Be knowledgeable in handling the traction and weights.
 Know how to move the patient on bed with their traction.
 Oral meds are given in the morning at 8am along with med card for proper identification.
 IV medication is given as per schedule. Use gloves when preparing and giving to patient. Use cottonswab to
disinfect the IVTT site prior to administration.
 NGT feeding with OF must be done accordingly and supervised.
 Wound dressing is done by the assigned Ortho Doctors. Offer assistance.
 S-pin site dressing/care are done daily- use 2 betadine balls to clean from site going outward to the pin. Then
replace the sterile gauze. Strict aseptic technique.
 Observe proper body alignment of the patient to straighten the skeletal traction. Keep traction device hanging
freely. The foot of the patient must not lean on the bed frame.
 Blood transfusion must be double checked with 2 RNs.
 Bed bathing, bed making and daily hygiene is a must.
 WARD CLASS on 2nd and 3rd DAY duty. 4 members in each day. Prepare your topics and materials.
 3 NCP , 3 Drug study for 3 days.
1 NCP per day,
1 Drug study per day
1 Physical assessment per day – – @ hotel. Time TBA
 1 case presentation – due at 3rd @hotel. Time TBA
----1 case that is significant to you. Have a complete assessment and documentation.
 Long Post test on 3rd day @ hotel. Time TBA.
