HOSPITAL SIMULATION

EMERGENCY ROOM

1. Patient will act on the signs and symptoms stated in the case scenario given - he/she
will be rushed to the ER, where the ER nurse will receive and assess the patient.
2. ER nurses will then conduct a thorough health assessment according to the patient's
signs and symptoms.
3. Endorse the patient to the physician and carry out doctor's orders.
4. ER nurse will then write the initial assessment and nurses notes.
5. Give initial interventions and medications.
6. Prepare the patient for transfer to Ward.
7. Endorse the patient to charge the nurse.

ADMISSION IN GENERAL WARD

(CHARGE NURSE)
1. Receives call from Admission Office or Emergency Room
-Receives preliminary telephone endorsement of patient’s:
 Name
 Available room number
 Conditions and amenities needed
 Chief Complaints
 Procedure Done
 Attending Doctor
2. Prepares room for Admission
-Rechecks room for readiness:
 Turn on the air conditioning, light, TV, refrigerator, etc.
3. Receives patient from Admission or ER
-Welcomes patient with a smile and introduce self
-Take the patient’s weight and height and ask for allergies. (If not done in ER)
Note: Take into consideration the manner that the patient is endorsed. For stretcher-borne
patients who cannot stand, weight-taking is exempted. While the height shall be
measured through a caliper.
-Receives the following from the Admission/ER:
 Patient’s Name Tag
 Admission Kit
 Discharge plan form (If the patient is ready for discharge)
 Neurological observation chart (For patients with neuro monitoring)
 Fluid Intake & Output
 Monitoring flow sheet
 HGT monitoring and Insulin administration sheet (For patients with HGT
monitoring)
 Admission and Discharge Record
 Consent properly signed
 History and Physical Examination
 TPR sheet
 STAT Medication Record
 Medication Record
 IV Fluids sheet
 Doctor’s Order Sheet
 Nurses Notes
 Diet list
 Surgical pathology
-Checks on the ID wristband and name of the patient & the completeness of the
Admission Kit.
-Ushers patient and relatives to the room
4. Renders preliminary Nursing Care:
 Provides privacy.
 Ask relatives to leave the room unless they will assist in undressing the patient.
 Helps the patient change clothes to the patient’s gown
 Assist him into a comfortable or required position in bed.
 Take care of the patient’s clothing and valuables. Explains hospital policies on
safekeeping.
 Orients patients and relatives to hospital policies and usage of room amenities
(Room Checklist)
 Takes vital signs
 Make an initial assessment of all pertinent observations and information gathered
from the patient and or family during the admission interview.
 Carries out admitting doctor’s order.
-Charge Nurse will carry out the orders
-Call the physician to inform that patient is in the ward.
-Update any changes from the patient
-attend to the patient’s concern or problem
-inform the patient that the physician will do rounds
-assist the physician during rounds
-relay to the physician the laboratory and diagnostic results.

5. Endorsement
-Endorse the medication ordered:
 Patient’s name and room number
 Attending physician
 Diagnosis and chief complaint
 List of ordered medications (name of medications, frequency, duration and route)
-The medication Nurse will administer the ordered medications on time and report any
changes in patient status to the charge nurse.
-Medication nurse will prepare the medications and medication tickets, give the
medications and plot them in the chart.
-Calculate medication dosages, write prescription and drug study.

6. Endorsement
-Endorse the patient’s current status:
 Patient’s name and room number
 Attending physician
 Diagnosis and chief complaint
 Patient condition
 Latest vital signs
 Monitoring data, such as vital signs, intake and output, neuro, HGT, etc.
-Bedside nurse will take the initial vital signs and monitor them.
-Plot to V/S at the TPR sheet
-calculate, change, and regulate IV fluids
-calculate input and output
-inform the charge nurse of any changes in the patient’s status
-write discharge plan.

7. Retrieves old charts from the Medical Records per Doctor’s order.
8. Documents nursing care rendered

OPERATING ROOM TO GENERAL WARD

(CHARGE NURSE)
1. Receives call from Operating Room
-Receives preliminary telephone endorsement of patient’s:
 Name
 Room number
 Conditions and amenities needed
 Chief Complaints
 Procedure Done
 Attending Doctor
2. Prepares room
-Rechecks room for readiness:
 Turn on the air conditioning, light, TV, refrigerator, etc.
 Check availability of oxygen tank if needed

(OR NURSE AND CHARGE NURSE)


3. Endorsement
 Patient’s name
 Room number
 Diagnosis and chief complaint
 Procedure performed (operation site, drainage output, etc.)
 Latest vital signs
 Latest doctor’s order (IV fluids, new medications, etc.)
 Medication given at the OR (remaining and used meds)
 Special endorsement such as precaution measure, presence of tube/drainage
such as JP drain,chest tube, foley catheter, etc.
