Summer Camp Volunteer Consent Form

I, the undersigned, hereby acknowledge and understand there may be inherent risks associated
with participating in the Summer Camp Program organized by Three-Flavors Kids’ Club, including
all manners of injury, illness and property damage, risks associated with or contributed to by
natural and manmade terrain, climatic conditions, limitations of their own physical condition; the
failure of equipment and the carelessness of other participants and misjudgements on the part
of the organization staff.

In consideration of my child, ____________________________ participating in the Program, I:

1. Give my permission for my child to participate in summer camp program.


2. Am willingly assume full responsibility, and any risks of injury, for my child in connection
with my child’s participation in the program.
3. On behalf of my child, my other heirs, next of kin, executors, administrators and assigns
agree to waive all claims that I or my child have or may have in the future against the
directors, employees and volunteers.
4. Release and forever discharge the Releases from all liability from any and all personal
injuries, loss, damage, expense, death or other loss and liabilities due to any cause
whatsoever arising out of or in any way connected with my child’s participation in the
Program, including those that are foreseen and unforeseen, and those for negligence and
breach of any statutory or other duty of care.
5. Agree to indemnify and hold harmless the Releases from any and all liability, loss, claims
demands, costs and expenses, including legal fees, arising from my child’s participation in
the Program.
6. Further agree not to make any claims or take any proceedings against any person or
corporation who or which might claim contribution and indemnity or relief from the
Releases.

I confirm that I have read and have had sufficient time to understand each item in the
Waiver Form. I understand that by signing this document I am waiving certain legal rights.
