Procedure Rating
I. Pre-analytical  Wear complete PPE for the procedure
Rating System:
5=satisfactory performance
Patient Identification and Preparation:
3=needs improvement  Greet the patient
0=incorrect/did not perform  Introduce himself or herself to the patient
 Identify the patient
 Verify requisition, medications
 Explain the procedure to the patient
 Preparation of materials
 Sanitize hands and put on gloves.
II. Analytical  Disinfected area for skin puncture.
Rating System:
10=satisfactory performance  Punctured to a depth of 3 mm.
5=needs improvement  Started the timer immediately as soon as the first flow of blood appeared
0=incorrect/did not perform
 Wiped the first drop of blood.
 Placed the second drop of blood onto the glass slide.
 Blotted the flowing blood with filter paper every 30 seconds. (for
bleeding time)
 Passed the tip of the lancet through the drop of blood every 30 seconds
and checked for the formation of fibrin strands. (for clotting time)
 Stopped the timer as soon as the flow of blood disappeared. (for
bleeding time)
 Stopped the timer when fibrin strands were seen attached to the tip of
the needle. (for clotting time)
 Properly recorded and interpreted the result/s.
III. Post-analytical  Check the site and apply bandage.
Rating System:
5=satisfactory performance  Properly disposed of materials used.
3=needs improvement  Cleaned and disinfected the working area.
0=incorrect/did not perform
 Thank the patient and sanitized hands.
