DISCLAIMER
1. The existing online processing of requested student record/s is a joint project between the Treasury Department and the Office of the Registrar.
2. Online payment must first be received by the Treasury Department. Confirmation of the payment will be automatically displayed in the request portal once validated and will
have the status "PAYMENT RECEIVED".
The requestor may check the status of his/her application using his Transaction ID.
3. Please take note that request will only be processed upon the acknowledgment of Office of the Registrar with the official receipt number of the requester.
4. Applications are processed from Monday to Friday 8:00 AM to 12:00 NN; 1:00 PM to 5:00 PM (except holidays and suspensions)
5. Please be informed that the Office of the Registrar will not produce a physical copy of records for the following documents, hence, a courier tracking number will also not be
produced:
1. CGFNS Records (for BS Nursing ONLY) (Registrar OTR- 630 - Nursing CGFNS - 500) and
2. WES Records (AVAILABLE ONLY FOR USE for World Education Services - WES)
2x2 Picture with white background JPEG Required for PRC Transcript Application
Scanned Copy of Transcript or Diploma JPEG / PDF Required for Certified True Copy Application
Note: In the absence of copies of the Diploma and Transcript of records, you may instead request the original copy.

Pending The request has been lodged in the system, the applicant needs to finish the process, and payment should be verified first
Awaiting clearance from the accounting office (Note: as per the Accounting Office processing time 1-2 working days payment button
For Clearance
will only appear after the clearance of accounts)
The request has been lodged in the system and is waiting for the applicant to finish the payment via the Online Payment Gateway.
For Payment
(Please take note for option B, that payment button will only appear once the account has been cleared by the Accounting Office)

INSTRUCTIONS
STEP 1 : Online Application of Student Records
1. Fill out the needed information for your online student records request.
Proceed to STEP 2.
STEP 3 : Payment
Payment Options: Debit/Credit Card (Visa, Mastercard, JCB and American Express), Maya, QR Ph, GCash, WeChat Pay, ShopeePay
Option A- Applicant SHOULD upload a Valid ID before the payment button will appear
Option B- Applicant SHOULD upload a Valid ID and Clearance of Accounts should be acquired first before the payment button will appear. Please be informed
that Clearance of Accounts usually takes 1-2 Working Days.
Note :If the requestee failed to pay the neccessary fee within 7 days, the Online Student Records Application the requestee made will automatically be considered as CANCELLED.
1. Requester can track the status of his/her Online Student Records Application by accessing the website of the Office of the Registrar ( http://registrar.ust.edu.ph). Click
"TRACK STATUS NOW" that can be found at the website homepage.
2. Provide the "Transaction ID" and Birthdate of the applicant to track the status of the requestee's Online Student Records Application.
The following instructions are part of your safety net. Everyone is enjoined to read and genuinely comply:
If the owner intends to send the requested student records applied for with the Office of the Registrar to a third party, he/she must send
1. An authorization letter specifying the complete details of the receiver (name, complete address, relationship with the recipient), duly signed by the owner, bearing the
specimen signature reflected on the scanned copy of a valid ID card, and
Reminders: If the data are entered incorrectly and consequently lead to change of data, such request will be considered new request, hence, turn-around time for the processing of
document begins at Day 1. If requested document must first originate from the college, and then finalized in the Office of the Registrar, the number of days for processing of such
document (based on the website) would not be followed, thus, reasonable adjustment shall be necessarily observed.
Disclaimer: All student records that are requested for online processing but whose payment is not settled after 7 days, such requests are automatically cancelled.
