AGREEMENT OF SERVICE
KNOW ALL MEN BY THESE PRESENCE:
This AGREEMENT OF SERVICE made and executed by and between:

ARTUZ 101 PROFESSIONAL LIGHTS AND SOUNDS RENTAL INC , a professional light and sound provider, with local address at
56 Zamora Street, Asis 1, Mendez, Cavite, duly appointed herein by its official representative, Nelson Pendon, hereinafter referred to as the “FIRST
PARTY”.

_____________________________________________________________________, hereinafter referred to as the “SECOND PARTY”.

NOW THEREFORE, in consideration of the promises and the agreements hereinafter contained and intending to be legally bound hereby, the parties do agree as
follows:

1. The FIRST PARTY agrees to provide the professional light and sound equipments and services engaged by the SECOND PARTY, to be their official light
and sound supplier.

2. The FIRST PARTY has all the equipments for lights and sounds provided on the selected package by the SECOND PARTY, as the FIRST PARTY set-up
and operates for the (Event) on (Date) , at (Venue) located
at _.
3. The SECOND PARTY agrees that the FIRST PARTY will provide the allowed number of personnel depending on the number of equipments and service
applicable for the selected package.

4. The FIRST PARTY will provide the qualified personnel to operate all the equipments inclusive of the package by the SECOND PARTY. NO other
personnel outside the first party personnel shall operate the light and sound equipments without the express consent of the FIRST PARTY.

5. The FIRST PARTY shall operate and leave the equipments set-up for a maximum time of _5 HOURS_ FROM THE START OF THE RECEPTION
PROGRAM. Additional charges will apply for extensions after the fifth hour of the event amounting to P 2,000.00 per hour.

6. The SECOND PARTY agrees that each FIRST PARTY personnel are entitled of Crew Allowance,a equivalent meal allowance amounting to P400.00 per
head (good for 2 meal) is a required.The total amount of Crew Allowance will be added to the total amount of package availed.

7. The SECOND PARTY shall pay a reservation fee of P 2,000.00. Philippine currency, upon the execution of this agreement, as a NON-REFUNDABLE
deposit for the supplier service.

8. The SECOND PARTY shall pay the FIRST PARTY an equivalent amount worth 50% of the total selected package by the SECOND PARTY less the
reservation fee 60 days (2 months) prior to the date of the scheduled event. No 50% down payment on the said date is due to cancellation. Failure to do so,
a written letter, e-mail, or any form of communication will be required to confirm the said event.

9. The SECOND PARTY will pay the remaining balance of the total package at any days prior to or on-the-day (OTD) of the event, but not exceeding OTD of
the said event.

10. All payments stated herein shall be made without the need of formal demand with the FIRST PARTY. Failure on the party of the SECOND PARTY to
tender the said payment particularly that stated in No. 8 shall give cause to the cancellation by the FIRST PARTY of his obligation to the SECOND PARTY
without prior notice and any deemed forfeited.

11. Cancellation of this agreement is allowed provided that a notice in writing, e-mail, or any form of communication is made at least two (2) weeks before the
scheduled date of event. Failure to give notice by the SECOND PARTY to the FIRST PARTY, an amount equivalent to 50% of the total refundable
deposit will be deducted for supplier service. Otherwise, the total refundable deposit will apply.

12. The FIRST PARTY shall render its service to the best of their abilities and on the manner of expected professional service provider.

13. That in the event of the cancellation due to Acts of God, fortuitous event, or a national health crisis, the parties shall agree to move the event to a different
date, or otherwise, the SECOND PARTY shall be allowed to assign this contract to another party at the SECOND PARTY’S sole discretion.
