1. All drivers must call for dispatch!!! NO EXCEPTIONS!! 866-976-5454 or email [email protected] include the load number in the subject
2. We require at least *2* check calls from all drivers DAILY (1chk call-am &1chk call-pm) between the hours of 7AM to 5PM CST, you can also
email to [email protected] or a penalty and or deduction will be applied to the agreed rate/amount. NO EXCEPTIONS!!
3. Driver MUST check call when loaded and unloaded.
4. Appointment times may not be changed!!! NO EXCEPTIONS!!!
5. No Double Brokering Under Any Circumstances. It Will Result in Non-Payment. Assigned carrier MUST have their authority on the side of the
truck that is picking up and delivering the load.
6. We must have the actual driver’s phone number that is carrying the load. NO EXCEPTIONS!!!
7. By signing this *Load Confirmation*, carrier agrees to the above rate, instructions and requirements set-forth by Loadquest.net Inc.
8. All Detention requests must be made prior to start of detention in order to get paid
9. If Shipper and Receiver addresses on the Bill of Ladings do not match the tender, you must notify your Loadquest representative immediately
10. Do NOT dispatch your driver/truck until this agreement has been signed and sent back to your Loadquest representative.

11. Carrier Agrees for the above rate will preform transport services for Loadquest, Carrier Agrees and understands that this agreement is not a
trip lease and carrier is an independent contractor with its own ICC operating authority.
12. Carrier understands that all permits and tax’s are its sole responsibility
13. Carrier agrees that it maintains its own current cargo, liability, and workers comp insurance.
14. Any damages/shortages/overages need to be reported to your Loadquest representative immediately prior to leaving Shipper/Receiver NO
EXCEPTIONS. If truck leaves the shipper/receiver without notifying a Loadquest representative all claims will be the sole responsibility of the
carrier listed in this agreement.
15. Any damages to Cargo while the truck is in transit including but not limited to collisions, and natural disasters must to reported to
yourLoadquest representative immediately NO EXCEPTIONS
16. DO NOT PAY out of pocket for Load or Unload- Call 866-976-5454 or email your Loadquest representative immediately, Carrier WILL NOT be
reimbursed with out prior written approval from Loadquest
17. All loads must remain sealed with seal numbers recorded on the Bill of Lading/ Delivery Receipt.
18. Driver/Carrier must notify Loadquest of any accessorial’s at time of occurrence or payment will be denied.
19. Carrier’s acceptance of this Tender and these terms and conditions shall be made either by Carrier’s signature and return of the Tender or by
actual acceptance of the tendered shipment. By doing so, the person acting on behalf of Carrier represents and warrants that he/she has been or
is specifically authorized to accept this Tender on behalf of Carrier and to legally bind the Carrier to the terms and conditions of this Tender

DEDUCTIONS AND FEES **** LATE PICKUP/LATE DELIVERY: MAY RESULT IN RATE DEDUCTIONS** MISSED DELIVERY DATE WILL
RESULT IN RATE DEDUCTIONS EITHER PER DAY OR PERCENT OF THE TOTAL LINEHAUL** (whichever is greater). BOL’S AND POD’S
MUST BE EMAILED OVER TO [email protected] WITHIN 24-HOURS OF DELIVERY OR PENALTY OF $50/PER DAY WILL APPLY
