This has reference to Ms. XXXX T, 14 year old, pursuing her 10 th grade at
NAME OF School, (Place). The academic institution comes under Indian
Certificate of Secondary Education and follows the (ICSE, CBSE) syllabus. She
was found to have average intelligence. IQ measured by WISC is 97 which fall
under 92-102 confidence level indicating average intellectual ability.
NIMHANS SLD indicates the difficulty in processing the grade level
Mathematical problems which is manifesting as below grade level performance
in Mathematics and other subjects. She finds the subjects associated with
arithmetic problems difficult.
In view of this, the girl is recommended to avail benefits such as extra time
during examination and permission to use calculator for computation in
Mathematics and physics and all the benefits may be given as per ICSE
Board rules and regulations.

Ms. P speaks English, Hindi and Tamil which she has been exposed to since birth. The
language spoken in P’s home is Tamil. It was observed that the student’s speech was clear
and intelligible and the student demonstrated English proficiency.
BEHAVIOURAL OBSERVATIONS
Ms. P arrived on time for the assessment and was accompanied by her teacher from her
school. She appeared alert and oriented, and appeared to put her best effort into the testing
process. She was curious and excited about the assessment initially, and stayed motivated
throughout the testing process.
TEST FINDINGS:
The NIMHANS Index for Specific Learning Disability was administered to assess learning
problems that interfere with her academic achievement. Ms.P was administered and assessed
with Level II of the battery.
ATTENTION: She completed the two digit cancellation grid in 8 minutes and 29 seconds
with many errors of omission and no errors of commission. This was below the average range
for children her age and standard.
SPELLINGS: She was assessed for written and oral spellings for familiar words at easy and
intermediate, levels. The score acquired was 29 out of 38 words. An analysis of spelling
errors revealed inattention, phonemic processing of words as well as omissions (disheartened
= disheartned, dissecting = disecting, correspondent = coressponded). However, she is still
performing at an average level with respect to spelling.
WRITING: The written expression test indicated inadequate content with average
vocabulary and minimal grammatical errors. While formation of sentences appears to be
adequate, her letter formation (Letter ‘f’), grammar and punctuation can be improved upon.
Her writing speed is found to be moderate.
READING: She was given a passage to read of level 8. She could read level 8 with little to
no effort. An error analysis showed mild difficulties in picking up phonetic cues and ignoring
punctuation at places. She appeared to have difficulty reading uncommon words such as
‘corrugated, dissecting and agonizing’. However, she was generally able to read the entire
passage with ease and reads at Independent level of reading at grade 8 level.
DIAGNOSTIC ARITHMETIC TEST: Ms. P on the diagnostic arithmetic test was able to
solve most of the problems of graded addition, graded subtraction, graded multiplication and
graded division correctly. She was able to solve basic fractions problems although they were
incorrect. A better conceptual understanding of the same can enable an improvement in
arithmetic. She is also slow in computation. Her functioning level in Mathematics is
appropriate to the middle of VIII grade.

SUMMARY
Ms. P is a bright girl with adequate interest in academics. She speaks fluently. Her social
behaviour is normal. She requires to put a lot of effort while computation. Her reading is
independent and fluent at VIII Grade. Comprehension is found to be adequate.
