Physical Activity Consent Form-Physical Education 2023

Dear Parent/Guardian

Baliwag Polytechnic College offers a variety of physical activities as part of its Physical Education
courses. By signing this physical activity consent form, you are giving your child permission and
approval to participate in the activities listed below.
Physical Education 1: Physical Fitness. The student is exposed to various fitness tests,
exercise modalities, and training techniques.
Physical Education 2: Calisthenics and Rhythmic Activities. The active involvement
of the student is emphasized in dance exercises, dance presentations, dance routines, and
dance-related activities.
Physical Education 3: Individual and Dual Sports. It requires active participation in
sports fundamentals, trainings, and mini-game play in badminton and table tennis sports.
Physical Education 4: Team Sports. A student is required to participate actively in
sports fundamentals, trainings, and mini-game play in basketball and volleyball sports.

As per Article XIV, Section 19, of the 1987 Constitution, Baliwag Polytechnic College will provide and
promote physical education and encourage students to live positively and actively in health and
fitness.
