EMPLOYMENT CONTRACT

This Employment Contract is made and executed by and between:

Mary the Queen College of Quezon City Inc., an educational institution created and
existing under Philippine laws with address at Commonwealth Avenue cor. Road 44,
Diliman, Quezon City duly represented by its President, ELIAS S. CIPRIANO, herein
referred to as the EMPLOYER;

-and-

ANNA MAY YU BELMES, of legal age, and a resident Lot 1324 Santan Ext. Lumang
Bayan Executive Village Brgy. Mayamot, Antipolo City, Rizal, of hereafter referred as the
EMPLOYEE;

WHEREAS, the EMPLOYER and the EMPLOYEE wish to enter into an


employment agreement governing the terms and conditions of employment;

THIS AGREEMENT WITNESSETH that in consideration of the premises and


mutual covenants and agreement herein after contained the parties hereby agreed as follows:

1. POSITION. The EMPLOYEE is hired as a SCHOOL GUIDANCE


COUNSELOR for the Academic Year 2023-2024;

2. TERM OF EMPLOYMENT. The employment of the EMPLOYEE shall


commence the date hereof and shall continue up the last day of the Academic
Year 2023-2024 unless sooner terminated in accordance with the provision of
this Agreement or for just, valid and lawful cause;

3. COMPENSATION AND BENEFITS. In consideration of the services to be


provided by the EMPLOYEE during the term of his/her employment, the
EMPLOYEE shall be paid the amount of Twenty-Five Thousand Pesos (Php
25,000.00) per month. It is understood that the rule on “no work-no pay”
applies in this case. However, this is without prejudice to other benefits which
the EMPLOYER may provide in the course of the employment;

4. DUTIES AND RESPONSIBILITIES. The EMPLOYEE shall be employed


in the capacity as SCHOOL GUIDANCE COUNSELOR, with the duties and
responsibilities set forth in Annex “A” hereof which made an integral part of
this Agreement;

5. SUBMISSIONS OF PAPERS AND OTHER DOCUMENTS. At the


commencement of the employment, the SCHOOL GUIDANCE
COUNSELOR shall submit photo copies of his or her credentials with the
Human Resource Department which shall be a condition precedent before
his/her salary is processed.
6. PAYMENT OF SALARIES. The payment of salary of SCHOOL
GUIDANCE COUNSELOR shall be every 15th and 30th day of each month in
the form of cash or check, as the case may be. Provided, that if said days falls on
regular or special holiday, payment of salary shall be made the following
business day. Provided further, that, SCHOOL GUIDANCE COUNSELOR
shall be released of his or her salary unless he/she had submitted his or her
complete credentials with the Human Resource Department;

7. YEARLY EVALUATION REPORT. Before the end of every Academic Year,


the GUIDANCE COUNSELOR shall be evaluated for his/her proficiency in
his/her field of expertise. Likewise, his/her credentials shall be re-evaluated for
re-admission as GUIDANCE COUNSELOR;

8. REPORTING SCHEDULE. The School Guidance Counselor shall be


given a flexible work day arrangement. This means however that the Guidance
Counselor is still required to commit reporting work on regular basis. However,
the Guidance Counselor may compensate any missed schedule on a different
day provided he/she shall be able to complete his/her required days of
reporting.

9. SUBMISSIONS OF REPORTS. At the end of every assessment period the


Guidance Counselor shall submit the pertinent reports to the Registrar’s Office
within ten (10) days after every assessment. In case of graduating class or there
are graduating students, he/she shall submit reports of said class or student
within five (5) days immediately before the deliberation of the Board of all
candidates for graduation. A special arrangement for special assessments shall
be made for the said graduating class or students;

10. CLEARANCE. At the end of every Academic Year, the SCHOOL


GUIDANCE COUNSELOR shall secure a clearance from the Registrar
showing that he/she had already submitted the reports. Likewise, he/she shall
secure a clearance from the school Property Custodian for whatever school
property he/she is accountable. The submission of this clearance shall be a
condition precedent before the last salary of the SCHOOL GUIDANCE
COUNSELOR will be released to him/her or his/her authorized
representative;

11. COMPLIANCE WITH THE COMMISSION ON HIGHER EDUCATION


REGULATIONS/UNDERTAKING. Upon the signing of this Agreement,
the SCHOOL GUIDANCE COUNSELOR hereby undertake that he/she shall
comply with the existing CHED and LEB’s Rules and Regulation, Circulars,
Memorandum and other Administrative Issuances. He/She shall pursue his/her
Masteral degree on his/her field of expertise and if not presently enrolled in any
Masteral program he/she undertakes that he/she will take his/her Masteral
program for his/her field of expertise. Failure of the SCHOOL GUIDANCE
COUNSELOR to comply with the comply with the existing CHED Rules and
Regulations, Circulars, Memorandum and other Administrative Issuances and
his/her failure to meet minimum units of his/her Masteral programs shall be a
ground of his/her termination or denial of his/her re admission as SCHOOL
GUIDANCE COUNSELOR;

12. SEVERABILITY CLAUSE. If any provision, or any portion thereof, contained


in this Agreement is held to be unconstitutional, invalid or unenforceable, the
remainder of this Agreement, or portion thereof, shall be deemed severable,
shall not be affected, and shall remain in full force and effect.

IN WITNESS WHEREOF, the parties have hereunto set their hands this ___ day

EMPLOYMENT CONTRACT and they have acknowledged that the same is their free and
voluntary act and deed.

This instrument of EMPLOYMENT CONTRACT is consists of four (4) pages


including the page on which this Acknowledgement is written, each and every page thereof,
on the left margin, having signed by the parties hereto and their witnesses and sealed with my
notarial seal.
