AFFIDAVIT
(Re: Change Unit with Voluntary
Surrender of Old Unit)

I, ANARIZA C. MACAPOBRE, Filipino of legal age, married and a


resident of Purok Sandayong, Santo Nino District, Pagadian City, after having
duly sworn to in accordance with the law, do hereby depose and say THAT;

I am one a new owner of motorized franchised with MTFRB CASE NO.


PAG-2024-0143, with Motor Side Car No. 0391;

Prior to the renewal of the aforesaid motorized franchised, I used a


Kawasaki single motorcycle for the operation of the said franchised;

Due to wear and tear, the said Kawasaki single motorycle became
obselete and at present, I replaced it with Bajaj Trike vehicle with Engine No.
AZXWNF89777 and Chassis No. BX4NWF15493, bearing Plate No. 614JTR.
Hence, this affidavit;

I am executing this affidavit attest to the truthfulness of the foregoing


statements to support the change unit of the sidecar used in said franchised
and at the same time I voluntarily surrendered the said old unit and for
whatever legal purpose this may serve;
