First of all, I thank Allah almighty for enabling me to do the client deal. All

the credit for my success goes to my respectful teacher, Ms. Usra Ahmad, who

gave us the opportunity to do this case history. Which got us to learn a lot. I

also thank the honorable head of the department of psychology, Ms. Usra

Ahmad, for her motivating advice, which helped me accomplish this task. The

love, care, support, and motivation of my father during the study gave me the

courage to do this work. I also thank my family as a whole for their

continuous support and understanding when undertaking my case history and

Demographic history:

Mr. SK is 23 years old; he is the second member of the family. He is educated and
unmarried.

Presenting complain:

The client was 23 years old. He has been facing this problem for the last two years. I feel sad and
hopeless all the time. I've lost interest in things I used to enjoy, and I just can't seem to shake off
this feeling of emptiness.

Symptoms:

● Low mood
● Loss of interest
● Difficulty concentrating

History of present illness:

The client belongs to a lower-class family. Having some personal issues. He is experiencing a
feeling of sadness, but he is still tearing up easily over minor issues. Whenever there is an
opportunity for happiness, there is not much happiness. He expressed a desire for either the
world to come to an end or for his own life to end. He has attempted to harm or destroy himself.

Whenever he sees an accident or blood, he becomes anxious and panics. That's why he avoided
social gatherings and did not meet people.

Past psychiatric history;

He told himself that it would be 3 years of illness. I have been treated by many doctors,
and now I am treating with a therapist.

Development history:

As his elder sister reported, his birth as well as delivery were normal. He was born at the
hospital. When he was a wanted child, He was brought up by his mother and was breastfed.

Medical history;

N/A

Family history;

He has mental health issues in his family.

Social history;

He reported, I don’t talk to everyone; I’m introverted. I don’t attend most of the social
gatherings. He has lost his love, which has affected his mental health.

Mental state examination;

He was wearing normal clothes. His eye contact was so weak. He was not speaking to the
point; he was just moving around the main point.
Psychological assessment;

After taking his history, we applied psychological tests to him. We selected,

BDI test

Psychological test result;

After scoring his BDI test, we get his test score of 44. Which represents severe
depression.

Provisional diagnosis;

Persistent depressive disorder.

Co morbidity;

● Anxiety
● Stress

Principal diagnoses;

Persistent depressive disorder.

Case Summary;

Mr. . SK is 23 years old and is a seoul engineer. He is unmarried. He has had a mental
health problem for the last 2 years. He has symptoms of persistent depression. He used sleeping
pills in an overdose.
Treatment recommendation:

On the basis of test results and history, we recommended some therapies, which are the
following:

Cognitive behavioral therapy
