D. Brief Description of the Due to the overlapping of the equally important
Program/Projects/Activities activities, there are some competencies and
lessons which were not learned at once. This needs
to be simplified and needs to be given ample of
time to discuss. As a result, me as the teacher
needs to conduct a remediation to bridge the
learning gap.

An individualized worksheet was provided to each


learner. The teacher conducted remediation.
Through scaffolding, they were able to learn the
topic.

E. Objectives a. To bridge the learning gap of the students about


plants and animal cell.
b. Ensure that majority of the students improve their
test scores in the specified topic.

F. Key Results The learners who scored low were able to improve
their result.

The concept was effectively learned since


individualized, simplified and scaffolding was
applied.
