Function of NDP:
TASK 1: Conducts regular visits to priority households with health problems
TASK 2: Prepares health status reports of families/ households visited
TASK 3: Plans for appropriate interventions on the indentified health concerns of families/ households
TASK 4: Assist in the preparation of barangay health plans
TASK 5: Supervise, train, orient the BHWs and conducts regular monitoring and evaluation of various health
programs together with the MHO, PHN, and DMO
TASK 6: Participate in the implementation of health programs of DOH and the LGU in the community
TASK 7: Assists in the conduct of disease surveillance
TASK 8: Conducts health education, advocacies and training on different DOH programs
TASK 9: Prepares reports of home visits, follow up patients, track referrals and community activities conducted
TASK 10: Coordinates with the Public Health Associates to ensure timely data collection and reporting
TASK 11: Assists the MHO and PHN in the conduct of PIR, MDR, and Death/Mortality reviews
TASK 12: Participate in data gathering and response during health emergencies and disasters
TASK 13: Performs other related functions as may be assigned subject to the approval of the DOH
