1 APPLICATION FOR BIN CANCELLATION USING FORM
MUSHAK-2.4
1.1 Introduction
1.2 Prerequisites
For being able to cancel BIN, you should prepare these documents (for applicable
case) carefully base on selected criteria:
Required information and soft copy documents for attachment showing the
valid reason for cancellation.

1.3 User Guide
Access your Taxpayer Online Services Overview screen (by login, the system
will redirect you automatically to this screen). Select the Company name from
dropdown menu bar:

Click on the “Add Form” button.

After that click on “OK” button.
The form will be loaded and ready for being filled. There are some buttons on
the top of page, which are:
“Check” is for ensuring all data in the form are valid [1].
“Save Draft” is for saving the form for further work [2].

The sub-form will be loaded and ready for being filled. There are some buttons
on the page, which are:
1. Add – You can add row for inputting the information. You can add more
than one required information. Click “Add” button> System will show a new
row> Input the information.
2. Edit- You can edit any information in any row. Choose the line that you
want to edit> “Edit” button will be enabled> Click the “Edit” button> the field
will be editable> Update the information.
3. Duplicate- You can copy any information in a new added row from the
previous row. Select the row you want to copy> “Duplicate” button will be
enabled> Click “Duplicate” button
4. Delete- You can delete any row from the added row(s). Select the row you
want to delete> “Delete” button will be enabled> Click “Delete” button.
5. To back to main form, click Please click here to go back to main form.

1.3.3 Document Attachment
Attachments:
Taxpayers have to check on the required document name from the list for
which the documents have been uploaded.

Select required documents scan copy for attachment [1]. And click on “Open”
button [2]. Taxpayer will attach documents based on the requirements of
VAT Officer. (For example: Bank Statement).

1.3.4 Submitting Mushak-2.4
In Form Mushak-2.4, you may validate the information that you provided by
clicking on “Check” button.
In case there is some invalid data, error messages will be shown on the
bottom of the page in the alert box.
Click on red button to see the detail error message. After that user can click
on each error message in the list then the system will redirect user to exact field
to correct information and check again.

In case there is no invalid data, the system will display “No errors found”.
In case of no error, if you click on the “Submit” button, a confirmation box will
be displayed as below. Click on “Cancel” button if you want to check and
change information again. Click on “OK” button to submit form.

When taxpayer click on the “OK” button, the system will display the
confirmation “Submission ID xxxxxxxx submitted successfully” in the alert
box. Noted that “xxxxxxxx” is Form Bundle Number.
Note: After submitting the Mushak-2.4 form, it will move to VAT officer worklist.
VAT officer will review the BIN cancellation form and officer will approve/reject the
form. Before rejection taxpayer cannot do any kind of amendment into submitted
form. If VAT Officer rejects the Mushak-2.4 Application for Cancellation or
Temporary suspension of Value Added Tax Registration/Turnover Tax Enlistment,
then taxpayer can resubmit the form by amendment. But if VAT officer approves

Mushak-2.4 Application for Cancellation or Temporary suspension of Value Added
Tax Registration/Turnover Tax Enlistment, any kind of further amendment will
disable.
After submitting the form Mushak-2.4, you will receive an email notifying that
the BIN Cancellation is in process.
You will also get the pdf of Mushak-2.4 in “My Document” of IVAS account.

If the application of BIN Cancellation is rejected by VAT Officer, you will receive
an email notifying the rejection of BIN cancellation as follows:

After the suspension of BIN, VAT Officer will verify the documents and cancel
the BIN manually. If the BIN is cancelled by VAT Officer, you will receive an email
notifying of successful BIN Cancellation as follows:

After the suspension of BIN VAT Officer will verify the documents and cancel
the BIN manually. If the BIN cancellation request is rejected by VAT Officer, your
BIN will be changed from Suspended to Active and you will receive an email
notifying of rejection of BIN Cancellation as follows:

1.3.5 Hard Copy Submission Process
You need to contact with your VAT Office and submit “Mushak-2.4 Application
for Cancellation or Temporary suspension of Value Added Tax
Registration/Turnover Tax Enlistment” hard copy Form directly to the VAT Office.
VAT Office will do the further arrangement to input your hard copy Form in IVAS
system. The rest of the process of approval\rejection will be online and you will
notify via e-mail.
