CHECKLIST FOR ENTRY INTO ENCLOSED SPACES
This permit relates to entry into any enclosed space and should be completed by the Master, Responsible Officer and by all
persons entering the space.

A This permit fulfills the 'Risk Assessment' required for entering endosed spaces. If any item is marked as 'NO', it is essentiai that permission is
obtaned from the company and a safe system of work agreed prior making entry. Hazards as detailed in secion 9.143 and generic tem piate on
PARIS for encdosed space entry, are to be taken into account.
B. A senarate checkiist is to be filled for each compartment to be entered
C. Vaidity of this permit should not exceed 8 hours and the maximum tme the space can be left unattended shall not exceed 4 hours
Al previously issued 'Safety Permits' should be revoked immediately upon vessel's berthing and fresh pemit should be issued only after obtaining
permisSIon from port / terminal

Notes:
1. Atmosphere measurement:
a) For entry purp0ses, steady reading of all the following should be obtained:
Oxygen 20.9% by volume; Hydrocarbon cortent <1% of lower flammable limit (LFL Toxic gases NIL ppm.
b) Tests in specific toxic contarinants (e.g. Benzene or Hydrogen sulphide), should be undertaken depending on the
nature of the previous contents of the space.
c) in order to obtain a representative cross-secion of the space's atmosphere, samples shoukd be taken from several levels
and through as many openings as possible. Ventilation should be stopped for about 10 minutes before the pre-erntry
atmosphere tests are taken.
2. Personnel:
a) Responsibie ofieer and the person incharge of the entny team cannot be the sane perso.
b) Cadet or trainee shall not ernter the enclosed space.
c) Person entering the space (Minimum of 2persons to enter in tandem except where the person entering is in line of sight from
outside the tank in small tanks)
3 iurination: Pneumatic úghts, insirinsicaily safe torches/head lanps are alowed on tankers. In aditon, only 24V electric lamps can be
Used on non-tank vesses and use of 110v/220v ights is stricty prohibited.
4. Esch Portable gas detector shal be calibated prio to use.
5. Each Personal gas detector shal be calibrated prior to se. Each person entering the enclosed space shall don the personal gas
detector.
6. The breathing apparatus shallbe tested for its gauge &capacity of air suppÄy, low pressure audible alarm and the face mask under
positive pressure and not leaking.
7 Forvessels fitted with BWTS- Ballast tank entry is protstitedfor irst 24 hours (or otherwise speciied in the manufacturers
instrucions) after deballasing using BwTS. Ater 24 hours of de-ballasing (o otherwise specifted in the manufacturers
instructions), Enclosed space ertry procedures to be complied pior making man entry.
1. Important that the gas readngs taken from EACH opening /manhole is taken and recorded clearty. For ER tanks having more than 1

Substance or any ship specific action, then the "risk assessment template" needs to be exported to Offñce and approved copy obtained
from Office PRIOR to tank entry.
