PLEDGE OF ADMISSION

“In consideration of my admission to the Palawan State University and of the privileges of students in this institution, I
hereby pledge to abide by and comply with all the rules and regulations as laid down in its Academic Policies and
Rules of Student Conduct and Discipline.”

As grantee of R.A. 10931, I will take the following into consideration:

1. The grantee of R.A. 10931 (Universal Access to Quality Tertiary Education) understands
and gives consent to Palawan State University (PSU) or its agents appointed to
facilitate the submission of needed personal data to the UNIFAST pursuant to the
requirement of CHED. The grantee also consents that such personal information may be
transmitted outside of PSU for transactions in relation to RA 10931.

2. The grantee shall render the required return service before graduation of the
baccalaureate degree after having benefitted from RA 10931.

By completing this form, I give permission for my data to be collected by Palawan State University (PSU).
I am fully aware that PSU or its designated representative is duty bound and obligated under the Data
Privacy Act of 2012 to protect all my personal and sensitive information that it collects, processes, and
retains upon my enrollment and during my stay in the University.

I understand that my personal information cannot be disclosed without my consent and that the
information that was collected and processed relates to my enrollment. I further understand that it is to be
used by PSU to pursue its legitimate interests as an educational institution. Likewise, I am fully aware that
PSU may share such information to affiliated or partner organizations as part of its contractual obligations,
or with government agencies pursuant to law or legal processes. In this regard, I hereby allow PSU to
collect, process, use and share my personal data in the pursuit of its legitimate interests as an
educational institution.

In addition, I am likewise giving my consent/permission in favor of my


parents/guardian/representative or whoever is responsible in providing care for me to access, verify,
examine and or inspect my academic and scholastic records, school fees/accounts in the University, and all
matters that relate to my status as a student at the University.
