Rehabilitation and Long-term care. We embarked on a journey that dates back to the early 1970s
in gerontological nursing; which provided care to our senior citizens. Since the early seventies,
the proportion of the population for our elders has been growing steadily and significantly, this
caused the increase for geriatric nurses (Touhy et al., 2012). In this essay, we will highlight our
case with a brief description of the spices tool, past as well as present history of our patient, and
finally a tailored care plan. Throughout our experience during our long-term care rotation, we
encountered many humbling experiences as well as lessons in which we can apply to our

significant, accurate data. One of the guides which we used in this assignment was the “SPICES
Tool”. Fulmer SPICES is a tool that asses six common conditions for our older adults. This
includes sleeping problems, problems when eating and feeding, incontinence, confusion,
evidence of falls, and skin breakdown, hence the name SPICES. Upon using this tool, it gives the
health care providers a generalized background of the patient’s overall health status in order to
create a care plan for high risks patients, as well as identifies symptoms an elder may encounter
during hospital stays or home setting. Prevent the early signs of these symptoms can shortened
hospital stays, and minimize any drastic injuries to the patient. Before completing an interview, it
is always best to inform your patient on what you will be delegating as well as obtain consent.
Prior to conducting our interview with our patient, there were guidelines we needed to
follow to ensure nurse-client relationship was not overlooked. We first explained and gain
consent from our instructor about the assignment, we then explained to our patient the purpose of
this assigned and continued after consent was obtained and felt comfortable enough to share her
