Chief Complaints

Defective vision in both right and left eye for the past
4 years
History of Presenting Illness
H/o Defective vision present in both left and right eye for
the past 4 years which was
● Gradual in onset
● Progressive in nature
● Near vision better than distant vision
H/o Day-Night variation in vision present with day time
vision worse than night vision
Associated symptoms
No H/o Pain
No H/o Redness
No H/o Watering of eyes
H/o Glare present for the past 6 months
No H/o Coloured halos
No H/o Black spots
Symptoms contd…
H/o of frequent change of glasses present, used
since past 2 years, for both near and distant vision,
now not using for near vision
No H/o Diplopia/ Polyopia
Past History
He is a known case of Type II Diabetes Mellitus for the
past 18 years, for which he is on regular medication.
No H/o Hypertension
No H/o Asthma
No H/o Tuberculosis
Treatment History

No previous cataract surgery


No H/o Laser Procedures
H/o usage of topical eye drops before 4 years for 2 months
H/o spectacles usage present for past 2 years for both
distant and near vision. At present not using for near vision.
Family History

No Relevant Family History


Personal History

● Patient consumes Mixed diet


● No sleep disturbances
● Normal bowel and bladder movements
● Denies any habit of alcohol intake and smoking
General Examination
● Conscious, Co operative, Comfortable
● Well built, Nourished
● No cyanosis
● No clubbing
● No icterus
● No pedal edema
● No generalized lymphadenopathy

Head Posture - Normal


Facial symmetry - Symmetrical

Summary

A known case of Type II Diabetes Mellitus of age 48 with


chief complaints of defective vision in both right and left
eye, right greater than left, with H/o glare, day and night
variation of vision.
visual acuity :
right - CF close to face, with pearly white opacity of lens
left - 4/60, with greyish white opacity of lens and iris
shadow present
Diagnosis

Right eye - Mature Senile Cataract


Left eye - Immature Senile Cataract
Plan
Right eye Small Incision Cataract Surgery with
Posterior Chamber IOL implantation after
investigation.
