HOW TO FILE A COMPLAINT AGAINST A JUDGE

To file a complaint against a judge, complete this form and send it to the Commission on Judicial
Conduct at the address above. The information you provide will be used to evaluate and investigate
your allegations.

To learn more about the purpose and jurisdiction of the commission and the types of allegations it
can investigate, read the enclosed brochure or visit our website at www.azcourts.gov/azcjc. A copy
of the commission’s rules and the Code of Judicial Conduct can be printed from the website.

Under the rules approved by the Arizona Supreme Court, complaints may be made public at
the conclusion of their review by the commission or upon the filing of a formal complaint
against a judge. If a complaint is dismissed, all personal information will be redacted from what
is made public.

words what you believe the judge did that constitutes judicial misconduct. Be specific and list all of the
names, dates, times, and places that will help the commission understand your concerns. Additional
pages may be attached along with copies (not originals) of relevant court documents. Please complete one side

The Honorable Judge Jackson has demonstrated undeniable bias while placing an innocent minor child
in danger. The Honorable Judge Jackson does not follow Court rules, Arizona statutues or Arizona law.
The Honorable Judge Jackson has demonstrated unprofessional behavior saying the words "shit" and "
bullshit" during proceedings. The Honorable Judge Jackson conducted herself in this manner during a
UCCJEA conference with a Judge from another state! The Honorable Judge Jackson has spitefully made
orders which have taken away protection for Mother and Child victims of domestic violence and child
abuse. Mother and Child have rights as victims in the State of Arizona which Judge Jackson ignores. The
Honorable Judge Jackson does not afford parties due process and tramples on their civil rights. Whoever
is monitoring the Honorable Judge Jackson while she is on probation should be watching her more
carefully. While consulting with attorneys, 4 out of 7 attorneys I spoke with refused to touch my case
based solely on the fact the case is assigned to Judge Jackson. Please see the attached document filed
in the Arizona Court of Appeals that will describe in more detail the concerning conduct of the Honorable
Judge Jackson.
