The identification proforma has been designed to collect information on routine
immunization through Polio teams
Sheet for information on routine immunization

We observed some common errors while collecting data
through the proforma during previous campaigns. These can be
easily rectified if communicated in advanced with the teams.

Ensure complete information for polio teams are filled on each sheet so we can
easily identify children when vaccinating them

Incomplete information filled about the team, their


supervisors, and the day of data collection

Enter complete dates according to the format on the proforma

The proforma is filled with incomplete date

Only add a tick or a cross while filling out the details of antigen, only add the date
for the last antigen in the last column

The polio worker has added dates for all antigens

While entering information on the proforma, please ensure teams are aware of the
difference between date of administration and the due date for next antigen

the EPI card can be used
to get information such as
child name, father name,
date of birth, and one of
the phone numbers
Use the ‘Vaccination
Date’ to fill out the
proforma
