chemical contamination, etc.
4 Check sledge for cuts.

5 Check sling eyes for damage to the stitching, cuts, and general
damage.

6 Check seams for damage and deterioration.

7 Check for friction burns or brittle and shiny patches caused by


overheating.
8 Equipment is fit for use - Yes / No

9 Checked by Contractor Safety officer / Supervisor

10 Checked by SIEMENS Safety Officer / Site In Charge
