Behaviour Change Challenge Proposal Form
Behaviour Change Challenge Proposal Form
Type your responses to the following questions regarding your proposed Behaviour Change
Report Proposal directly into this form. You should submit this summary online using
Brightsapce. You must also attach a typed copy (or screenshot) of your self-monitoring sheet.
We will read these over to ensure you are on the right track. Please do not stretch this form
beyond 1-1.5 page (the amount of space provided is roughly the amount we expect you to use).
I would like to make sure I get into the habit of exercising daily. I aim to exercise for at least 30
minutes each session.
2. What kind of intervention will you use and how will you implement it? Be sure to describe
your planned intervention as well as the psychological term used to refer to it (e.g., positive
reinforcement, cognitive restructuring, implementation intentions, etc.). List ONE reference
(from a journal) in APA 7th edition style to support your selected intervention.

3. How will you measure the behaviour (provide log sheet and explain when/where/how it will
be used)?
This logsheet will be printed out and pasted on my door, such that I have to see it everytime I
leave my room. Each time I exercise in the week will be recorded down until the weekly quota is
achieved. Hopefully after Week 10, I will have the habit of exercising daily and the logsheet will
no longer be needed.

4. Personal reflection.
Personally, I would like to take this chance to get back the habit of exercising regularly. I have
been exercising on and off and since I am naturally a lazy person it takes quite some time for me
to pick up exercising regularly again while it is easy to drop it. As such, I hope I eventually
cultivate it into a healthy habit that I do not see as wasting time or difficult. As a student, I am
also constantly seated either writing or using a computer, as such exercising will be able to force
me to move around and not be so lethargic at home.
