STATUS PRAESENS

GENERAL PHYSICAL EXAMINATION:


Body length or height, body weight, gross body disproportions and malformations.
General appearance of the child: posture (kyphoscoliosis, crooked neck, asymmetry of the thigh creases),
position in bed (active, passive, forced)
Breathing: frequency, signs of dyspnea, type of dyspnea, accompanying audible phenomena (groaning,
moaning, stridor, wheezing,
Cough: quality of cough (hoarse, bitonal, stridor, dry, productive)
SKIN: color (pink, pale, yellowish, cyanotic), pigmentation, rash, pathological changes, distribution of
hairiness, skin smell (acetone in ketoacidosis, urea in uremia, mouse urine smell in a
phenylketonuric child).
SUBCUTANEOUS TISSUE: development of subcutaneous adipose tissue on the trunk, extremes
Circulation: frequency and fullness of the pulse, peripheral blood supply, signs of circulatory shock, TA.
Body temperature: axillary, rectal.
General impression of the severity of the disease (probably a healthy child, gives the image of a mild,
moderately severe or very severe patient. turgor and state of hydration, edema.
LYMPH NODES: palpability and size, consistency, mobility, soreness in typical places occipital, cervical,
submandibular, preauricular, sublingual, axillary, inguinal
MUSCLES: development, tone, symmetry
SKELETON: gross malformations and deformities, signs of rickets, signs of hip dysplasia

PHYSICAL EXAMINATION THROUGH DIFFERENT SYSTEM

HEAD: circumference (measure, ), shape (conspicuous, inconspicuous), fontanelles, craniotabes, hair


(density, color, shine),
FACE: symmetry, peculiarities of physiognomy, signs of dysmorphia.
EYES: position, bulbomotor, nystagmus, strabismus, hypertelorism, exophthalmos, endophthalmos,
photophobia, epiphora,conjunctival secretion, conjunctivitis, width, shape and reactions of the pupils, sclera
(color), transparency of the cornea and lens.
NOSE: Shape, passage, secretion, expansion of the nostrils during breathing.
MOUTH: lips, teeth, mucous membrane, tongue.
PHARYNX: palatal ports, tonsils (size, exudate), retronasal secretion, posterior wall.
EARS: position and shape of the ear, external ear canal, discharge, mastoid sensitivity,
NECK: symmetry, mobility, lymph nodes, goiter, pulsations, respiratory retractions of the jugular.
CHEST: shape, symmetry, deformations, intercostal indentations, breasts.
LUNGS: breathing frequency, signs and type of dyspnea, cough, percussion findings, auscultatory findings.
HEART: protrusion of the precordium, visible pulsations, palpable flow, auscultatory findings (frequency,
rhythm, tones, noises).

ABDOMEN: size, shape, visible peristalsis, venous pattern, navel, palpation findings (abdominal wall
resistance and sensitivity, liver, spleen, pathological resistance), peristaltic sounds.
GENITALIA :Male: penis and testicle size, prepuce, hypo/epispadias, cryptorchidism, hernia, hydrocele,
degree of development of secondary sexual characteristics.
Female: development of labia majora and minora, clitoris (hypertrophy), discharge, degree of development of
secondary sexual characteristics.
LOCOMOTOR SYSTEM: symmetry, deformations, paralysis, mobility, edema, swelling and mobility
of joints, hands (too many or missing fingers, syndactyly, clinodactyly, groove of four fingers, club
fingers), nails, flat or deformed feet. Spinal curvature (physiological and pathological), mobility,
neck pterygium, spina bifida, meningocele, myelocele, sensitivity of the lumbar region, spinal column or
pelvis.
NEUROLOGICAL STATUS:
CONSCIOUSNESS: preserved or demaged in the form of somnolence, stupor or coma.
Social contact, negativism and higher brain functions - the job of a clinical psychologist.
MENINGEAL SIGNS, MOTORS, GAIT, MUSCLE STRENGTH, TONE, REFLEXES,
COORDINATION, HYPERKINESIS, SOMATIC FEELINGS, SPECIFIC FEELINGS
(sight, hearing, balance, taste and smell).
