FIR SUMMARY FOR 81V5 INSPECTION PERIOD: 16-03-2023


to 26-03-2023
(MTBE – BUTANE DRIER) (FIR#230,308,395)

Observations and actions taken:


During the MTBE 2023 TA, The Vessel was offered for inspection.
Visual inspection (External & Internal) was carried out for the Vessel.

External:
 Name plate is found intact and readable.
 This equipment is insulated. Minor cladding damage near Manway M1
was observed, however overall Insulation cladding condition found
satisfactory.
 Grounding connection found in satisfactory condition.
 Platform, ladders, gratings, Lifting Lugs and foundation found in
satisfactory condition.
 Minor paint damage at lifting lugs was observed.

Internal:

 Manway M1 fasteners found with superficial rust.


 All nozzles internal neck found free from choking.
 Hold down grating found in satisfactory condition.
 Support screen and retaining device plate found intact.
 Shell barrel 1 & 2 internal surface observed covered with thin layer of
yellowish process scale deposits.
 Top head and shell barrels # 3 & 4 internal surface found clean and free
from major abnormalities.
 Thick corrosion scales and paint damage was observed on nozzle ‘B’,
Manways # M1, M2 & M3 and associated covers internal mating flange
bolting areas.
 Top nozzle cover M1 internal surface including associated nozzle ‘B’
piping found with blackish deposit
 Bottom head internal surface found covered with debris.
 Mechanical cleaning was performed for all internal weld joints including
longitudinal, circumferential, nozzles & attachments and found without
any major weld defects visually.
 Visual inspection carried out for all remaining location after re-cleaning
and found as satisfactory.

NDT:

 DPT was performed after mechanical cleaning on all internal weld joints
including seams, nozzles and internal attachments and no relevant
indication noticed and found as Accepted.
 UT thickness performed for remaining bottom head TML and result found
Accepted.

Repair carried out during TA: Nil

Any Future Recommendations: Nil
