Note :- No Proposed saving declaration is required from employees whom opted New Tax Slab.

Proposed saving declaration only by those employees who have opted Old Income Tax Slab is
mandatory.

I the undersigned hereby undertake and declare that the following amount has been/will be paid by me
during the Financial Year 2024-25
and proof of the same shall be submitted to your office before 31 st Oct. 2024 and that I shall be fully
responsible for all the consequences , if the declaration is found incorrect at anytime .

1. House Rent paid from ___________ to __________ @Rs. _______/- Per month (Original
receipt(s) of the same is enclosed) .Copy of PAN card of Landlord should be attached with
declaration and also PAN number should be mentioned on Rent receipt. Full address of

2. Accrued interest from house loan _____________ Residential house, proof must be attached
viz Possession/Completion Certificate , copy of permission / NOC of Railway Administration , if
the payment is being paid to other then railway .Copy of accrued interest & Form No. 12 C (Self
Declaration of Loss/Gain) to be filled up.

3. Rebate of 40% and above Handicapped employees (Handicapped Certificate must be attached).
In case employees ward 100% handicapped , certificate of the same is to be attached and also
certificated of expenditure spent thereon to be attached; Otherwise rebate will not be
permissible.

4. Rebate on Mediclaim Policy u/s 80D of Income Tax Act Rs. ______________for the year 2024-
25. Proof must be attached.
5. INVESTMENT UNDER SECTION 80 C (Max, Rebate Rs. 1, 50,000/-). Life Insurance premium Self ,
Wife, Minor Children ( Year 2024-25) , proof must be attached.

1. Option for Income Tax Slab is mandatory .


2. No declaration and proposed saving shall be accepted after 31.10.2024
3. The declaration without mentioning of PAN No. will not be accepted and Form No. 16 will
also not be issued.
4. Landlord’s PAN No. is mandatory for claiming Rebate of House Rent.
