HSE INTRODUCTION

 Accident - Undesired circumstances which give rise to ill-health or injury, damage


to property, plant, products or the environment; production losses or increased
liabilities.
 Incident - Undesired circumstances and ‘near misses’ which could cause accidents.
 Unsafe act : any activity by workers which are not as per the prescribed safety
standard or practice and which can cause or likely to cause accidents or risk for
self or others at workplace, damage equipment's and bring losses in terms of
reputations and revenue to employer.
 Unsafe Condition : hazards that have the potential to cause injury or death to an
employee. Some of these hazards include erroneous safety procedures,
malfunctioning equipment or tools, or failure to utilize necessary safety
equipment such as goggles and masks.
 Hazard : Any source of potential damage, harm or adverse health effects on
something or someone.

 The Halifax Explosion (Canada) – 2,000 dead, 9,000 injured
 The Bhopal Gas Leak (India) – Up to 16,000 dead
 Honkeiko Colliery Mining Disaster (China)– 1,549 dead
HISTORY OF HSE in Malaysia

 The Bright Sparkler in 1991


 The Bright Sparklers Fireworks fire occurred in Sungai
Buloh, Selangor, Malaysia on 7 May 1991
 he Bright Sparklers Fireworks Factory in Sungai Buloh, Selangor caught fire
and caused a huge explosion.
 26 people were killed and over a hundred people were injured in the disaster.
 The explosion was strong enough to rip the roofs of some local houses, and
ended up damaging over 200 residential properties.
Cause of the accident

 The nature of the lab workers who perform work that has been completed to
test the fireworks to be sold
 Attitudes of employers - During fireworks test is running , the workers are not
monitored by the management of the laboratory
 Working environment that is not secure , have been operating for 13 years but
did not have a complete fire prevention equipment
Impact of the Accident

 26 people were killed and 103 people were injured


 50 rescuer had breathing problem
 Destroyed factory, sawmills, food coloring, animal, plant and firecracker
 Rescue unit : 8 fire fighter's car , 2 lories of army.
 Medical unit : 9 ambulance from HKL , 5 ambulance from HSB

 OSHA Act has bee approved on the parliament on 1994

BENEFIT OF OSHA

 OSHA protects workers by stressing the importance of safe practices on the


job because without an understanding of potential dangers on the work site
workers become at higher risk for a workplace injury or death.

PENALTY

A person who contravenes the provisions of section 15, 16, 17 or 18 shall be


guilty of an offence and shall, on conviction, be liable to a fine not exceeding
fifty thousand ringgit or to imprisonment for a term not exceeding two years or
to both.
