I am pleased to have you use this tool for your personal use. By using this software you recognize that it is created by Dr. Jeffrey Moore and
it is copyrighted. For further research in this area please visit:
http://andersonuniversity.libguides.com/content.php?pid=404406
1) Please go to the yellow self-assessment tab and fill out the 20 questions. After doing this your results will appear in the Results tab. To
compute your profile go to the five profiles boxes and enter your data after assessing your results in reference to the charts and graphs.
2) Please give 4 or 5 people that you work with on a weekly basis the peer survey on the peer tab. You can copy the 20 questions and email them
or print them off and hand them the printed survey. In order to guarantee that there will be as little bias as possible let them know that their
feedback is to be anonymous. You can do this by having people email a secretary the information or put the hand written survey in your office box
or given to a 3rd person. When you receive all the peer surveys you need to enter their scores for each question in the green peer tab. The
document will auto compute the average score for each question and then auto populate into the results tab.
3) Please give your supervisor the Benchmark survey. This is NOT your supervisor's evaluation of your work but rather the supervisor
EXPECTATION for your current job. Enter your supervisors scores in the red benchmark tab.
4) After the peer data and the supervisor benchmark data has been entered your results will be located on the black results tab.
There is no charge for the use of this software. By the end of 2013 I will have this tool automated online. I will be adding a peer component
for you to have others evaluate you. You will then be able to compare your rankings to your peer's perspective.
Kind regards,
Dr. Moore
Listed below are some statements that describe cultural and managerial preferences. You are asked to answer based on your best judgment of your preferences.
Indicate how often you engage in these behaviors, using the scale of 1 (almost never) to 7 (almost always) to respond to each statement. Please place a number
from 1 to 7 for each question. There are not "right" answers. However if you skip a question the spreadsheet automatically scores it a zero.
