NOMINATION UPDATION

1. Photo copy of the shareholder’s Pan Card duly self-attested with date.
2. Photo copy of the shareholder’s Aadhaar Card duly self-attested with date.
3. Photo copy of the Nominee’s Aadhaar & Pan Card duly self-attested with date.
4. An application requesting for updation of Nomination in the share certificate.
5. For Adding of Nomination details kindly fill the Form no. SH 13.
6. For Opting out of Nomination kindly fill the Declaration Form ISR 3.
7. For Cancellation or Variation of Nomination kindly fill the Form no. SH 14.
