DECLARATION

 I / we certify that all the information furnished by me / us above is true to the best of our knowledge,
information and belief.
 I / we shall furnish all other information that may be required by SHPI / SIDBI in connection with my / our
application and I/we have no objection to your furnishing the information submitted by me / us to any agency
as you may deem fit in connection with consideration of the assistance.
 I / we have no objection to MUTHOOT MICROFIN LTD (name of SHPI) / SIDBI / its representatives making
suitable enquiries and checks with Banks / Credit Bureaus / RBI/ other persons while considering the
application. I / We further consent to MUTHOOT MICROFIN LTD (name of SHPI) / SIDBI carrying out
identity and fraud prevention checks and sharing information relating to this application for fulfilling Know
Your Customer (KYC) norms of SIDBI.
 I / we do hereby agree to accept MUTHOOT MICROFIN LTD (name of SHPI) as a representative of SIDBI
in matters of credit, repayment and follow-up etc.
 I / we agree to repay the loan amount as per repayment schedule which may be fixed by the Bank.
 I / we hereby agree to the terms and conditions and also agree to abide by any amendments to the terms
and conditions, as may be stipulated by SIDBI, from time to time.
 I / we hereby declare that I / we are not defaulter(s) to any Bank / financial institution.
 I / we undertake that the funds will be utilized only for the purpose as mentioned in the loan application
form.
 I / we hereby authorize the Bank to disclose all or any particulars / details / information relating to our loan
account with the Bank to any other Banks / Financial institutions / Government or any other agency as may
be considered necessary or desirable by SIDBI.
 I / we hereby agree to execute requisite documents as may be required by SIDBI for availment of loan.
 As per my / our loan application request, if SIDBI considers the loan application and sanction loan, I / we
request you to kindly disburse the loan amount of ` ___________/- on or before _________ to the
disbursement / pool account of ………………(name of SHPI) who is acting as a service provider of Small
Industries Development Bank of India (SIDBI) for further disbursement to my / our bank account indicated in
