SALARY CASH ADVANCE REQUEST LETTER

September 6, 2024

Dear Sister,

Greetings! I, Alvin V. Rubio, Computer and Robotics Teacher, am writing this letter to
make an official request for a Cash Advance of 10,000.00 for the Medication of My Son, He is
now suffering illnesses since last week. Due to the urgent nature of his Heart condition, I have
been advised to proceed with a specific medication that is critical for his treatment.

I would be very grateful if you could deduct that amount on a monthly basis of Php. 8,000.00 by
October, and Php. 2,000.00 by November 2024.
