name and date of birth? 2) My name is Tim Johns, and my date of birth is 08/01/1989. 3) Can you tell me what is going on with you? 4) Just this morning when I was leaving my house, I started to experience a chest pain. And my wife brought an ambulance to get me here. 5) So, were you worried very much when that had happened? 6) Yes, Doctor, Too much. That was my very first time to experience that sort of pain. 7) And, can you tell me more about the pain? 8) Yeah, it was a pain in my chest. The whole episode of the pain lasted about 14 minutes. And the paramedic gave me some painkillers to ease the pain. 9) You took the right decision by coming in here since you have been worried about it. What I will be doing right now is asking you some questions to be able to figure out what is going on with you. 10) Sure. 11) So, can you firstly describe the pain? 12) As I told you, it was here in my chest. It did move out to my arm and abit to my nick as well. 13) Apart from that, did the pain radiate somewhere else? 14) No, Not really. And the pain felt like a pressure on my chest. 15) Could you say that the pain started suddenly or gradually? 16) I would say it came out all of a sudden. 17) Did the pain lasted for the whole 14 minutes? 18) Yeah, the pain stayed for the whole 14 minutes. It was in a form of a steady pain. 19) Was anything made the pain lighter? Like changing your position? 20) Not really. 21) Anything made the pain worse? 22) No, it was really quite bad all the time long. 23) I am trying to figure out the intensity of the pain. On a scale from one to ten with ten being the worst. How would you rate the pain? 24) It was quite high, it was 8 or 9. 25) And How is it for the time being? 26) It is much better, I would say it is 5 or 4. 27) If you felt that the pain starting to go worse, I can ask one of the nurses to get you more painkillers. So, please let me know if anything changes. 28) Ok, I will. 29) I have asked you a lot of questions, And I now have an idea of what could be going on. But, I would love to ask you about your prospective? 30) I am really worried. I remember my dad having the same symptoms when he was 55 years old. He had to go a bypass surgery. I hope I would not go through that myself. 31) Yeah, I feel you. Did he recover well after that surgery? 32) Yeah, I think he did. He seemed very healthy after that operation. 33) I have more questions to ask, Can i? 34) Sure Doctor, please go ahead. 35) Do you have recent fever, Nausea, vomiting or diarrhea? 36) Not that I know off? 37) Any recent weight loss or poor appetite? 38) Do you have any heartburn reflexive acid or any shortness of brith? 39) Yeah, some reflux actually. 40) Are you diabetic or do you have blood pressure? 41) Yeah, I do have blood pressure. My primary care physician asks me to check it quite often. 42) Well, please relax here. I would like to listen to your heart. And I will order an ultrasound to check your heart.
