SECTION B RELEASE

THIS RELEASE is in respect of claims under Section B -Accident Benefits of the Alberta
Standard Automobile Insurance policy (SPF 1) .
IN CONSIDERATION of the payment of the all-inclusive sum of $1350,00
I, Jamie Martial (hereinafter referred to as the "Releasor")
1. hereby remise, release and forever discharge Primmum Insurance Company, its insurers,
heirs, executors, administrators, , employees, agents, officers, directors, successors and
assigns (hereinafter referred to as the "Releasees"), of and from all actions, causes of
action, suits, debts, claims, demands, losses, damages, interests, costs, expenses and
compensation , whatsoever at law or in equity, known or unknown, which the Releasor
had, now has or can, shall or may hereafter have, against the Releasees or any of them,
by reason of or resulting from Section B- Accident benefits coverage contained in the
policy of insurance issued by Primmum Insurance Company as policy number
00140576353 arising out of a motor vehicle accident which occurred on or about October
29, 2023 in Boyle Alberta.
2. IT IS UNDERSTOOD AND AGREED that this settlement does not constitute an
admission of liability on the part of the Releasees, and that such liability is specifically
denied.
3. IN CONSIDERATION OF this settlement the Releasor covenants and agrees that the
Releasor nor anyone else to the Releasor's knowledge has caused or will cause to be
commenced any legal proceedings in respect of any matters released herein.
4. IN CONSIDERATION OF this settlement, the Releasor undertakes and agrees not to
make any claim or take any proceeding against any other individual, partnership,
association, trust, unincorporated association, corporation or person who might claim
contribution, indemnity or other relief over from the Releasee in respect of any matters
released herein. If any such claim is made, the Releasor covenants that he will indemnify
and hold harmless the Releasees of and from any claim that may be brought against them.
5. IT IS FURTHER UNDERSTOOD AND AGREED that the Releasor has been given an
opportunity to seek independent legal advice prior to the execution and delivery of this
Release and that the terms of this settlement are fully understood that the amount stated
herein is the sole consideration of this release and that such amount is accepted
voluntarily as a full and final settlement of the claim for claims specified above.
IN WITNESS WHEREOF, the Releasor has hereunto set his hand this ___ day of
