SECTION A (40 MARKS)


Answer ALL the questions in this section.

1. Record keeping requires all stakeholders to participate in the process. Identify FOUR
responsibilities employers have related to OSHA record keeping (4 Marks)
2. Methuselah is injured by a machine while trying to feed a workpiece. Highlight FOUR
information about this incident that should be captured in an incident occurrence report.

3. As an occupational safety and health officer, list FOUR occupational safety and health
records you should keep at a workplace. (4 Marks)
4. It is always good to know where to find information. Enumerate FOUR resources inside
the workplace that will help you find information on safety and health issues. (4 Marks)
5. Explain FOUR indicators of hazards that an occupational and health officers should be
monitoring in the pursuit of ensuring a safe workplace. (4 Marks)
6. Imagine a tank containing a poisonous gas has started leaking in a factory. List
contingency measures you will use to minimize the casualties of the incident. (4 Marks)
7. Discuss FOUR importance of occupational safety and health risk evaluation in a
workplace (4 Marks)
8. In an emergency of fire in a workplace, describe FOUR procedural steps involved in
handling the incident. (4 Marks)
9. Identify FOUR methods of gathering occupational safety and health issues raised by
workers. (4 Marks)
10. First aid is always the first thing done to victims of accidents. Explain FOUR reasons why
first aid is important to the victims of workplace injuries. (4 Marks)

SECTION B (60 MARKS)


Answer Any THREE Questions in This Section.

11. Company A, nowadays, has frequently been found to have several cases of accidents and
injuries in its production section.
a. Discuss FIVE likely causes of these accidents and injuries. (10 Marks)
b. Discuss FIVE categories of safety gears, based on the part of the body it protects,
that employees in company A should use. (10 Marks)

12. After risk assessment of a machine in Blueway company, the occupational safety and
health committee recommended that it is dangerous to human operator due to the physical
hazards it has.
a. Discuss FOUR physical hazards that are likely to be found in the machine.
(8 Marks)
b. Considering each element of hierarchy of risk controls, suggest, for each, what
Blueway company should do. (12 Marks)
13.
a. Three incident reports were recorded at company XYZ in a span of four days. Risk
assessment was conducted and it was noted that their machines were working well
and were safe and the likely cause of the incidents was overexertion and tiredness
faced by the employees. Discuss FIVE unhealthy administrative arrangements that
may have led to this. (10 Marks)
b. Discuss FIVE rights given to employees by occupational safety and health.
(10 Marks)

14. Workplace is an environment where a person is doing his or her work. Depending on the
nature, different workplaces possess different factors which may contribute negatively or
positively to the safety of whoever is working there.
a. Describe FIVE factors that contribute to unsafe workplace conditions. (10 Marks)
b. Discuss FIVE steps involved when conducting risk assessment in a workplace so as
to minimize factors that negatively affect safety. (10 Marks)
