APPLICATION FOR EXCHANGE OF BLASTING CERTIFICATE


OF COMPETENCY
INSTRUCTIONS:
1. Refer to the attached payment options and submit original proof of payment for R110,00 with this application.
2. The applicant is required to apply in the Region established by the Minister of Mineral Resources & Energy (DMRE) in
terms of section 47(2) of Mine Health and Safety Act, where the mine falls under its jurisdiction.
3, DMRE Regional office contact details attached.
4. Telephonic enquiries can be directed to the Regional office of the DMRE where the mine falls under its jurisdiction.
5. The following originally certified copies from the South African Police Service (SAPS) not older than six (6) months
must be attached to this application and arranged in the following order:
• Identity Document or Passport
• First Aid Certificate recognized in terms of Minerals Act, Regulation 24.7, in force in terms of schedule 4 of the Mine
Health and Safety Act, 1996 (Act No. 29 of 1996) as amended
• Certificate of Fitness to perform work at a mine issued by the Occupational Medical Practitioner (OMP) appointed in
terms of section 13(3)(a)(i) of the Mine Health and Safety Act, 1996 as amended
• Gas testing and flameproof certificates, if applying for fiery mines
• Record of service on mine’s letterhead with contact details as proof for qualifying number of shifts (indicating start, end
date and the number of shifts per competency/outcome) as required by Minerals Act (MA) Regulation 28.41.3 in force
in terms of schedule 4 of the Mine Health and Safety Act, as amended
6. The original copy of the Provisional Blasting Certificate of Competency.
7. The application must be endorsed by the Manager appointed in terms of section 3(1)(a) of the Mine Health and Safety
Act.
NB: Incomplete documents will not be considered and will be returned to the applicant

Applicants may pay the required amount at any ABSA Bank, Account Number 406176 9154 and use
the examination centre where they will be writing their examination as the reference number. The
reference numbers are as follows:
