Drug formularies

Drug formulary system:


The process whereby a health care organization establish policies on use drug products and identify drug products and therapies that are most appropriate and cost effectiveness to best serve the health of agiven population
Drug formulary((list of drugs))
Continually Updated list of medication, relating information representing the clinical judgment of physician, pharmacist in diagnose & ttt of disease,
Application it is applied into ambulatory care setting, inpatient facilities.
Objectives are to provide:
Information of Therapeutic information about each item Special information about
1-drug products.approved for use 2-institutional policies that govern drug use. drug (dosing,abbreviation….)

Definition It is an advisory committee responsible for developing. And updating the drug formulary system It consists of 1-physician, 2-pharmacist 3- other health care providers
It is responsible for:

substitution of drug products that contain same (active ingredient) & chemically identical in strength, drug products contain different (active ingredient) which are of the same pharmacologic / therapeutic
conc, dosage form and route of administration class. And are expected to produce the same therapeutic effect ex piroxicam and other NSAIDS

: Method of using therapeutic alterative:

3-Compare efficacy & frequency of SE among therapeutic alternative efficacy
4-assessing the impact of drug product on patient compliance 2-Evaluate products acc. To their impact on total health care cost.

Contents of drug formulary


1- Institutional policies & procedures: 2- Drug product list (the heart):
1-Description of P & T committee, membership& responsibilities. Generic name
2-Procedure for addition & deletion to formulary. Common synonyms & brand name
3-Regulation for prescribing & administration of drug. ‫الضوابط‬ dosage form including strength,packaging and size
Formulation & all active ingredient
4-pharmacy policies as hour of service, orders ,processing ,labeling, packaging& drug distribution
Additional information include:
1-Usual Adult, pediatric dosage 2-cost information 3-Special Caution (refrigerate,.or IV administration.) 4-controlled substances class
