Accidental

Exposure
Immediately following the exposure:

1. Wash needle sticks and cuts with soap and large amount of running water
2. Irrigate eyes with sterile irrigant, sterile saline, or clean water
3. Flush splashes to the nose or mouth with water

• Do Not squeeze the wound.


A. The source (patient) is tested for:
1. HBsAg
2. Presence of HCV antibody
3. Presence of HIV Ag/antibody combi

B. The exposed person is tested for:


1.HIV Ag/Ab
2. anti-HCV,
3.HBsAg
4. anti-HBs titre and anti-HBc Total.
5. Pregnancy test.
• For HBV:
- If the dentist is vaccinated, and a known responder, there is no risk of
transmission.
- Response unknown after one complete series test anti-HBs, if <10
mlU/ml, should receive HBIG & initiate re-vaccination then test for anti-HBs.
- If the dentist is vaccinated, known non responder after 2 complete
series, should receive HBIG within 48 hours, and another dose after 1 month.
- If the dentist is not vaccinated or incompletely vaccinated, should
receive HBIG within 48 hours, and complete vaccination schedule.
- If source is unknown, manage like positive source.
• For follow up, after 3 months test for HBsAg, anti-HBs, anti-HBc

• For HCV:
For follow up, test for HCV-RNA at 3 weeks for early diagnosis.
- If positive, start HCV treatment.
- If negative, test for anti-HCV after 3 months, then 6 months.
For HIV:
- In high-risk population start treatment immediately initiated within 72 hours of
exposure, preferably within 2 hours, to be most effective. Four weeks course of 3
antiretroviral drug therapy.
For follow up:
- 3 weeks post exposure test for: HIV Ag/Ab,
- 6 weeks post exposure test for: HIV Ag/Ab
- 4 months post exposure test for: HIV Ag/Ab.
- 6 months post exposure test for: HIV Ab
