 Hoist Operators must be well trained by a competent person of its
Serious Injury, controls, functions and safe use (Do’s & Don’ts) prior to appoint as
Supervisor,
01 Untrained Operator. Property Damage, 3X5 operator. L

Fatality.  Hoist operators must have valid 3rd party certificate from Local Authority
approved agency.
 Do not use defective hoist. Use only after defects is rectified.
 Ensure proper inspection and periodic maintenance is carrying out as per
schedule / manufacturer instruction.
Serious Injury,  Pre-start inspection checks must be carried out on a daily basis prior to

02 Defective Hoist Property Damage, 3X5 start using. (Pre-start checklist). L

Fatality.  Use the hoist only if all the controls and safety switches are in normal
operational condition. (Do not by-pass any control devices).
 Hoist must be stopped if any unusual vibration, sound or any other
abnormalities monitored.
 Hoist must be certified by Local Authority approved 3rd party after initial
erection, Jumping, any major repair or once in six months.
 Certificate showing SWL and number of person allowed must be

03 Over Loading. Property Damage, 3X5 L
 Do not over load the hoist under any circumstances. Foreman

 Do not rely on overload cut off switch; only allow the permitted load on
hoist.
 Ensure certified safety device on all hoist used for carrying person.
04 Fall of person / Materials. Serious Injury, 3X5  Hoist cage must be fitted with interlocking doors. Supervisor, L
Property Damage,  Before starting the Hoist ensure all doors are properly closed and Foreman
Fatality. passengers are not touching the doors and no materials kept against the
door.
 Sound and safe (Fully boarded, standard edge protection and sufficient
support) landing platform should be provided.
 Landing platform access must be keep closed with gate that should only
be opened from outside. (Hoist side).
 Do not allow anyone to lean or touch hoist doors while hoist running.
 Loose / small items must be carried on bags / boxes and do not keep any
materials against doors.

 Do not keep any loose materials / rubbish on top of the cage, always
maintain good housekeeping.
 All people must be on queue while boarding / leaving the hoist.
 Operator should frequently monitor the hoist way for any obstructions.
 Floor edge permit system to be implemented strictly and do not allow

 Hoist must be parked safely on the ground and power supply to be Supervisor,

 All control panels should be protected from rain water / fog. Operator
 Hoist and its all controls should be thoroughly checked prior to resume the
normal operation.
 Ensure proper inspection and periodic maintenance is carrying out as per
schedule / manufacturer instruction.
 Pre-start inspection to be done prior to start using the Hoist.
 Power cable slackness must be periodically checked and adjusted as per
manufacturer recommendation.
Serious Injury,  Cable guides to be provided as required and damaged / missing cable Supervisor,

07 Material Damage, 4x4 guide & rubber guard must be replaced immediately. Foreman,
Cable while running.
Fatality.  Operator and supervisor should regularly monitor the cable and ensure it Operator
is in correct position.
 Hoist operation must be stopped in heavy wind condition (above 45km/h)
and ensure there is no foreign particles (Paper, clothes, rope etc)
entangled on the cable prior to start.
 Hoist must be fitted with Safety Device with a valid 3rd party certificate.
08 Rescue while unexpected Serious Injury, 4x4  Do not use hoist in adverse weather conditions. (Heavy wind- above Supervisor, L
breakdown. Fatality. 45km/h, Rain, dense fog, poor visibility etc.) Foreman
 Hoist must be stopped and serviced if any unusual vibration, sound or any
other abnormalities monitored.
