Likelihood (capability of frequency and avoidance): 1 = Rare; 2 = Remote; 3 = Occassional; 4 = Frequent; 5 = Almost Certain
Risk Assessment : Severity (severity of result, seriousness of accident): 1 = Negligible; 2 = Minor; 3 = Moderate; 4 = Major; 5 = Catastrophic
Risk Level = Likelihood × Severity = 1~25

Judgement of Risk Level:

1~3 = Low Risk, no additional risk control measures may be needed but conduct frequent review to ensure the accuracy of the risk level over time
4~12 = Medium Risk, carry out careful evaluation of the hazards to ensure the risk level is reduced as los as is practicable within a defined time period
15~25 = High Risk, not acceptable and must be reduced to at least medium risk before work commences; eliminate hazard before work commences if needed
