1. REFERENCE: Blotter Entry No. 2024-03-00123 dated March 7, 2024 at Legarda


Police Station, Baguio City

2. WHAT: Possible violation of Art. 294. Homicide

3. WHERE: Legarda, Baguio City

4. WHEN: At about 2:30 PM.of March 7, 2024

5. WHO:
Suspect: Juan Tamad, wearing yellow plain shirt.
Victim: Crisostomo Ibarra, male, single, 25 years old, delivery rider, and
residing at Legarda, Baguio City.

6. WHY: To identify, apprehend, and file necessary charges against the suspect.

7. HOW: On March 7, 2024, at around 2:30 PM, in Legarda, Baguio City, Juan
Tamad, wearing a yellow plain shirt, purposefully and illegally shot Crisostomo
Ibarra twice in the head. Following a playtime session with the victim, the
suspect was defeated in a "Basketball" match. Furious, the suspect stood up,
took a revolver out of his pocket, and shot the victim in the head. Subsequently,
the suspect swiftly left the crime site. Upon being taken to the hospital, Ibarra
was pronounced "dead on arrival." Officers arrived on the site at around 2:30
PM on the same day. The SOCO Team was called to conduct the crime scene
processing.

SPECIMEN/S SUBMITTED:
1. Two (2) fired bullets specimen recovered from the body of the victim with
markings RAF/030724-1 and RAF/030724-1
PURPOSE OF LABORATORY EXAMINATION:
-To determine whether or not the two (2) specimens were discharged from one
firearm.

FINDINGS:

- Preliminary investigation of the two (2) bullets through physical examination


revealed that they have congruent class characteristics – 9 mm caliber, six (6)
lands and grooves, and right twist of rifling.
- Microscopic examination revealed that the two specimens possess matching
individual characteristics as manifested by the ballistic marks present on both
bullets thereby concluding that the two (2) bullets were fired from the same
firearm. (See attached comparison photomicrographs of the specimens)

CONCLUSION:

- The Questioned Specimen 1, Specimen 2 with markings RAF/030724-1 and


RAF/030724-2 recovered from the body of the victim on March 12, 2024
Specimen are POSITIVE IDENTIFICATION and were discharged from the same
firearm with 9 mm caliber, six (6) lands and grooves, right twist of rifling.
REMARKS:

- TIME AND DATE COMPLETED: 04:00 PM, March 16, 2024


- The four specimens were released to PSSG. VEGETA (Crime Laboratory
Custodian) on March 13, 2024
- A copy of this form was given to authorized officer and an original copy will
remain in the laboratory for future reference
